We are looking for a Provider Affairs Representative to help support our growing operation which provides exemplary, highly coordinated primary health care in the right place, from the right provider at the right time .

The NB Health Link program leverages technological solutions and innovative, patient-centered care models to provide access to primary care services, both virtually and in person.

The Provider Affairs Representative plays a key role in supporting the seamless transition of patients between providers and ensuring accurate, timely updates to patient records. This position is responsible for coordinating patient list distribution, managing provider transitions, and maintaining strong relationships with internal teams and external partners such as the Primary Healthcare Access Coordinator (PHAC), Regional Health Authorities (RHAs), and provider offices. Additionally, the individual also supports the onboarding providers in the program and their satisfaction during their tenure with the program. The representative also contributes to continuous improvement initiatives and supports the development of operational tools and documentation.

Role & Responsibilities: The Provider Affairs Representative shall perform tasks including but not limited to those identified below:

Patient and Provider Transition Coordination

  • Coordinate patient transitions such as de-rostering and patient matching.
  • Ensure timely and accurate updates in the EMR.
  • Liaise with PHAC, RHAs, and provider offices to support smooth transitions.
  • Assist in the administrative tasks related to provider onboarding
  • Track and monitor transition progress and escalate issues as needed.
  • Oversee the distribution and reconciliation of patient lists to and from providers.
  • Ensure data integrity and timely updates in the EMR and the provider recruitment pipeline
  • Maintain accurate records of list returns and provider responses and provider onboarding

List Management

Partner Engagement

  • Build and maintain strong working relationships with Providers, PHAC, RHAs, and clinic administrators. Represent the program in meetings and workgroups related to provider transitions.
  • Support conflict resolution and facilitate collaborative problem-solving.

Process Improvement & Program Support

  • Participate in program expansion and continuous improvement initiatives.
  • Identify workflow inefficiencies and recommend solutions.
  • Participate in the drafting and review of policies, procedures, and work guides.
  • Support the development and implementation of new tools and processes.

Documentation & Reporting

  • Prepare, edit, and format reports, correspondence, and program documentation.
  • Provide regular updates to leadership on transition activities and outcomes.
  • Maintain organized records and contribute to data analysis efforts.

As the ideal candidate for this position, you:

Education & Experience:

  • Post-secondary education preferred, ideally in health administration, business, or a related field, or an equivalent combination of education and experience.
  • Minimum of 3 years' experience in a healthcare, administrative, or coordination role.
  • Experience working with Electronic Medical Records (EMR) and mass data imports and exports.
  • Experience coordinating with multiple partners.

Knowledge & Skills:

  • Strong understanding of patient-centered care and provider workflows.
  • High attention to detail.
  • Demonstrated analytical and problem-solving skills.
  • Proficiency in Microsoft Office Suite (Excel, Word, PowerPoint).
  • Excellent organizational and time management skills.
  • Ability to work independently and collaboratively within a team.
  • Effective communication skills with the ability to tailor messages to different audiences.
  • Proven relationship management and partner engagement skills.
  • Strong sense of professionalism, accountability, and service orientation.
  • Bilingualism in French and English is required.
  • Ability to work remotely and adapt to a dynamic, evolving work environment.
  • Ability to work in a high pace and dynamic environment
