POSITION PHILOSOPHY


Under the direction or supervision of the RN, LPN, UC, and/or CM the CCA is responsible for creating a human habitat, surrendering institutional routines, and providing care and services to meet the needs of the residents in the cottages. The CCA is committed to alleviating the three plagues of loneliness, helplessness, and boredom through facilitating small, unexpected pleasures in the day-to-day life of the residents. This is based on the established policies and procedures, philosophy, mission, vision & values of Rosecrest Communities and the Eden AlternativeTM. The CCA works cooperatively and respectfully with residents, coworkers, families, visitors, and volunteers to create a positive and pleasant environment in our community.

POSITION DESCRIPTION

The Continuing Care Assistant (CCA) functions as part of the interdisciplinary care team under the supervision and direction of the RN, LPN, UC, and/or CM. The CCA supervises, assists with, or performs personal care for the residents and assists them with activities of daily living while promoting independence and autonomy through encouragement, respect, dignity, and support.

PERFORMANCE REQUIREMENTS

CCA Standards/Scope of Practice:
The CCA will adhere to the Scope of Practice for CCA's as developed by the NS provincial CCA Professional Advisory Committee. The CCA adheres to the scope of Employment as set out by Rosecrest Communities.

  • • Establishes and maintains an individualized approach while applying communication strategies and techniques in all situations.
  • • Collects data to support the client assessment.
  • • Demonstrates person-centered communication with residents, family, and the care team.
  • • Follows CCA Standardization by working to full scope.
  • • Practices in a manner consistent with the principles and legal requirements for maintaining a resident record.
  • • Contributes to the development and ongoing revisions of the resident-specific, holistic plan of care, as a part of the collaborative care team.

Personal Care

  • · Treat residents with empathy, dignity, and respect at all times.
  • · Request and accommodate residents' individual choices whenever possible.
  • · Respect the spiritual and cultural beliefs and practices of the residents.
  • · Encourage and support residents to maintain or regain independence.
  • · Respond to resident requests as quickly as possible.
  • · Provide personal care according to care plan/Kardex.
  • · Support relationships that are central to the resident.
  • · Assist residents to have clothes purchased, laundered and repaired as needed.
  • · Facilitate the maintenance of the resident's personal effects according to the wishes of the resident.


Maintenance of the Home

  • · Organize the residents physical environment according to their specific wishes and needs.
  • · Assist the resident to personalize their room.
  • · Clean resident areas following facility standards.
  • · Making Beds.
  • · Light housekeeping.
  • · Report shortages, breakage, and malfunctioning of equipment.
  • · Implement infection control measures.

Continuous Improvement

  • · Participate in quality monitoring in Rosecrest as required.
  • · Attend and participate in education programs aimed at increasing personal knowledge and abilities of the caregiver, and those that also lead to the certification for special procedures required at Rosecrest.

Safety

  • · Work to create a culture of safety for all.
  • · Ensure safe operation and maintenance of equipment.
  • · Participate in mandatory safety education.
  • · Participate in safety processes in cooperation with the JOHS committee.

Emergency Planning

  • · Know and adhere to Fire, Emergency & Security policies, and procedures

Communication

  • · Use a positive and respectful tone of voice when conversing with residents, families, and staff.
  • · Proficient in verbal, written and electronic forms of communication.
  • · Maintain confidentiality of information about the resident and his or her family, etc.
  • · Keep the nurse informed of any change in resident condition through verbal report, documentation and incident reports as needed. Reports must be timely based on the type of change observed.
  • · Ask for guidance and consultation from other team members.
  • · Participate in care conferences for the residents.
  • · Accurately record resident care and observations on the clinical record of the resident (Point of Care)
  • · Responsible for ordering supplies for cottages from Food and Nutrition Services through established methods.

Confidentiality

  • · Maintains confidentiality of all personal and health information for staff and Residents of Rosecrest Communities.

Creating a Human Habitat/Resident Comfort and Support

  • · Maintain safety of the resident environment
  • · Organize resident environment to accommodate personal choice and independence.
  • · Actively participate in Resident committees or programs.
  • · Answer call bells.
  • · Escorts residents to and from programs.
  • · Delivers mail, reads mail to residents if assistance is needed.
  • · Aids with technology and communication with families.

Education

  • · Attending and actively participating in educational and mandatory programs in order to increase knowledge and abilities.
  • · Ensures Surge Learning mandatory education modules are complete and up to date.

Cooperation & Flexibility

  • · Work with peers and residents/families in a problem-solving approach.
  • · Demonstrate ability to be flexible in hours of work that facilitate work life balance and meets resident care needs.
  • · Follows "book off" procedures for any absences.
  • · Adheres to all Rosecrest Policies.
  • · Surrender institutional routines while ensuring quality of care is maintained.

Other related duties

Other related duties may be required to fulfill our commitment to providing a "human habitat" such as but not limited to:

  • · Share your talents and interests with the residents and coworkers in keeping with Rosecrest professional boundaries guidelines.
  • · Facilitating and participating in activities related to children and pets.
  • · Accommodating resident requests for activities that enrich their lives.

QUALIFICATIONS

  • · Nova Scotia Certification as a Continuing Care Assistant.
  • · Registered and in good standing with the Provincial CCA Registry.
  • · Proven experience in a LTC setting an asset.
  • · Demonstrate ability to alter work on a regular basis.
  • · Good oral and written communication.
  • · Work in an ever-changing environment, and be prepared to adapt to these situations in a respectful and understanding manner

HOURS OF WORK

Rosecrest Communities provides service 24 hours a day, 7 days a week. Hours of work will reflect operational requirements with the goal of providing safe Resident care.
