Job Summary

As the First Link Intake Coordinator, you will report to the Manager of Programs and Services and work on-site at our Resource Center, joining a team dedicated to supporting clients and their care partners. In this role, you are part of a team that provides direct client services to persons living with dementia and/or their care partners. You will be responsible for managing and processing all referrals from the community, including those from healthcare professionals and self-referrals; entering client information into the system; ensuring all documentation is complete; triaging client needs; recommending applicable programs; and completing referrals to ensure the timely delivery of client services.

They will have the ability to work collaboratively to accomplish various tasks and manage multiple projects. The successful candidate will invest in building community relationship and establishing meaningful connections with our clientele.

Typical Duties and Responsibilities

  • Complete intake screening and entering data into the database, assessing the client's needs, and triaging accordingly
  • Manage and implement triage process for direct and self-referrals
  • Intake and registration for all programs and services where appropriate; including all database documentation, and appropriate communication to program staff
  • Support and promote a culture of inclusion and belonging by fostering a respectful environment in every client interaction, while contributing to ongoing diversity, equity, and inclusion initiatives
  • Send information packages to families and individuals who contact the office for support and information
  • Participation in all staff, department, working group and other meetings as appropriate
  • Support the Programs and Services department with work under the referral partner strategy
  • Maintain confidential client files
  • Lead, maintain, update and add resources to internal community resource library
  • Work to expand awareness to the general public through various awareness activities
  • Participate in provincial and national activities when requested
  • Travel as required

Qualifications:

  • Must be bilingual in French and English. Unilingual candidates will not be considered.
  • A 2-yr diploma in health or social services field and Office Administration, or equivalent;
  • Knowledge of Alzheimer's disease and other dementias, community services, healthcare, the not-for-profit sector
  • Community awareness with the ability to build trusting relationships with community partners
  • Sensitive to the impact of Alzheimer's disease on the individual and the family
  • Demonstrated knowledge and application of person-centered approaches, problem-solving strategies and confidentiality;
  • Must embody the vision and values of the ASNB
  • Must be approachable, have solid communication skills and a warm positive attitude with active listening skills
  • Must be able to forge strong partnerships with like-minded organizations in the community to build awareness and strengthen the organization
  • Ability to work well independently and as part of a team
  • Proficient in Zoom and Microsoft programs
  • Ability to work outside regularly scheduled hours as required
  • Experience working with databases;
  • Intake experience an asset;
  • Professional phone experience an asset;
  • Possess a valid NB driver's license and be available to travel within the region
