Duties: This position will operate out of the Humber Valley White Bay Family Care Team hub site in Deer Lake, the incumbent will be providing services across multiple program areas including virtual care clinics. This is an advanced professional level position involving a high degree of responsibility, autonomy and independence. The Social Worker will work collaboratively within the Family Care Team in facilitating intensive case management of individuals accessing services across the continuum of care.
Within policy parameters, the Social Worker accepts, screens, and actions referrals in relation to individuals who require supportive service. The successful candidate will provide case assessment and planning as well as emergency intervention. The Social Worker 1 will ensure person and family centered care that wraps available resources around the individual wherever the individual resides. The Social Worker will establish a preventative, integrated, person and family centered approach driven by individualized assessment of needs and client individual choice to promote aging in place. Work involves carrying an active caseload of clients in receipt of an array of services. Working directly with clients/families/care providers, social workers continually assess and monitor client strengths, resources, needs, and approved services. The Social Worker is part of an interdisciplinary team responsible for Primary Health Care services based on the needs of the population in the health neighborhood. This can include the provision services in a variety of settings and methods including in person visits, virtual care. and visits in clients homes including within personal care homes.
Qualifications: The successful candidate must hold current registration with the Newfoundland and Labrador Association of Social Workers as well as an undergraduate degree in Social Work. A minimum of three years current experience (within the last five years) in a community or hospital based setting. The incumbent must demonstrate strong written and oral communication skills. The ability to establish and maintain effective working relationships is essential. Experience working independently with limited supervision and a willingness to travel are also required. Mileage and appropriate per diems will be paid in accordance with the NAPE HP Collective agreement.
