Under the direction or supervision of the Director of Care, the Care Navigator is responsible for creating a human habitat and providing care and services to meet the needs of the elders in the cottages. The Care Navigator works cooperatively and respectfully with residents, coworkers, families, visitors and volunteers to create a positive and pleasant environment in our community.

Position Description

The successful candidate will have the opportunity to guide residents, families and caregivers through the continuum of care ensuring seamless transition, coordinating services, and advocating for residents' needs. This role involves providing education, facilitating communication between care teams, and supporting residents in accessing appropriate resources for optimal health and well-being. The Education piece of this role will support continuous professional development, ensure compliance with provincial standards and promote best practice in long term care , ultimately contributing to improved resident outcomes.

Responsibilities

  • Reports directly to the Director of Care. Works collaboratively with the Clinical Care Manager and Assistant Director of Care.
  • Facilitates admission and in-house transfers for residents in close collaboration with the Assistant Director of Care (ADOC) and Clinical Care Manager; as well as other team members.
  • Conducts pre-admission and on-going assessments of residents' bio-psycho-socio-cultural needs. Working in consultation with the ADOC and Clinical Care Manager.
  • Addresses issues and challenges presented by and/or regarding residents and family members through individual support/counseling, group work, etc.
  • Investigates complaints related to resident abuse, and/ or reports critical incidents as directed by the Director of Care, Clinical Care Manager or Assistant Director of Care.
  • Coordinates and facilitates multi-team meetings family council, palliative care
  • Keeps accurate and timely documentation following facility processes.
  • Maintains accurate statistical record as assigned.
  • Creates community partnerships that will benefit the residents.
  • Plans and implements interventions for challenging behaviors in collaboration with internal and external resources.
  • Chair or sit on various committees to promote resident and staff safety.
  • Provides staff support and education as appropriate. Provide debriefing as required.
  • Liaison with (but not limited to) Department of Health, hospital discharge planners, placement officers, DCS/ Community Social Workers, and school and other community partners.
  • Liaison with School and other outside partners to coordinate education and other developmental needs for residents within the Rosecrest Community.
  • Facilitates special needs requests for residents as appropriate.
  • Advocates for the best interest of residents.
  • Organizes and facilitates care conferences.
  • Develop tools / kits / pamphlets, and other informational materials that will benefit residents /family members and staff.
  • Familiar with advance directives, power of attorney, and other pertinent legislation that may impact residents in long term care. Able to assist families in navigation of processes to identify these key people.
  • Participates in ongoing professional development and lifelong learning.
  • Experience working with all ages preferred, geriatric experience considered an asset.
  • Other duties as assigned to support Rosecrest Communities.

Qualifications

• Diploma in Practical Nursing, licensed and in good standing CRNNS or Bachelor/Diploma of Social Work, licensed and in good standing with the regulatory body.

• Be well versed in dealing with people with various backgrounds and personalities.

• Strong understanding and advocate of best practices in healthcare

• Strong computer and analytical skills

• Must have the ability to deal with complex situations and make sound recommendations

• Ability to network and develop strong relationship with team members and community partners.

• Strong oral and written communication skills

• Strong conflict resolution skills and Resident centered care as the core motivator for service delivery.

• Certificate in Gerontology or working experience with the seniors' population is an asset

Maintains confidentiality of all personal and health information for staff and Residents of Rosecrest Communities.

Creating a Human Habitat

Maintain safety of the resident environment

Organize resident environment to accommodate personal choice and independence.

Actively participate in Resident committees or programs

Education

Attend and actively participate in educational and mandatory programs in order to increase personal knowledge and abilities.

Assesses and facilitates staff education needs.

Apply for educational funding as appropriate.

Assist with managing SURGE Learning.

Other Related Duties

Other related duties may be required to fulfill our commitment to providing a "human habitat".

Hours of Work

Rosecrest Communities provides service 24 hours a day, 7 days a week. Hours of work will reflect operational requirements with the goal of providing safe Resident care.
