We're looking for a Licensed Practical Nurse to supervise a dynamic Homecare team who is interested in an opportunity to Lead, mentor and inspire a team in Yellowknife, Northwest Territories!

Your day:

  • You lead, mentor, and inspire a team of 10+ Personal Support Workers delivering essential services to clients in their homes.
  • Starting at 8:00 AM with the main Homecare huddle.
  • Leading the morning PSW huddle (8:30-9:00 AM) to review client needs and staff workloads.
  • Conducting in-home client intakes and referrals for services like personal care, respite, meal prep, and Elders in Motion.
  • Creating and updating care plans, ensuring safety and quality of service.
  • Supervising PSWs in the field, addressing challenges, and supporting continuous improvement.
  • Collaborating with the Homecare Manager and Clinical Supervisor to align care strategies.
  • Mentoring PSW students, managing schedules and payroll, and leading weekly staff meetings.
  • Participating in Continuing Care meetings and working groups to shape the future of community care.

Compensation and Select Benefits:

  • You provide the Care; we provide the Car; vehicle provided for patient visits.
  • Salaries: $96.8k - $115.7k
  • Paid benefits: $ 24.9k - $ 27.9k paid benefits in the first year

This job typically requires a Licensed Practical Nursing Diploma and 3 years of recent experience in a home or community health care setting as a Licensed Practical Nurse and 2 years of recent supervisory experience are required. Able to obtain a Licensed Practical Nurse license in the Northwest Territories with CANNN. Current CPR certification and maintained annually. First aid is current and maintained as per the organization's expectations. Equivalent combinations of education and experience will be considered.
