Title: Senior Coordinator, CHW, PATH Program

In this role, you will:
- Assess client needs on eligibility to access the PATH program according to established policies and procedures.
- Onboarding new clients for program by reviewing program offerings, parameters, any associated costs, etc.
- Play an active role in researching, identifying, developing and implementing new or re-aligned services/initiatives
- Liaise with community to determine vulnerability and gaps in program/services.
- Liaise with other community agencies/organizations regarding related services/initiatives to foster mutual understanding, coordination and partnerships.
- Develops implementation plans for program delivery
- Evaluates service delivery, present findings and recommendations for improvement
- Assists in creating service/initiative goals and objectives
- Provides on-site support to management of physical assets (premises, vehicles,
security) - Provides on-site management support of financial assets and activities (petty cash, cash receipting)
- Assists with preparing and monitoring program specific budgets, completes program statistical reports, assists with grand submissions, fee-for-service initiatives, and assists with preparing progress reports.
- Assists with review and analysis of budget variances
What we are looking for :
- Minimum of 3 years of experience and a 1-2 year post-secondary certification/diploma in gerontology or social services, or less experience if combined with a 3-year college diploma or university degree.
- Ability to identify and resolve issues, by applying standard procedures and functional or organizational knowledge.
- Communication skills, diplomacy and empathy to interpret information and deliver advice, clarify client or stakeholder needs, offer solutions, and resolve escalations in new or somewhat sensitive situations.
- Relationship-building skills with community members, partners, and stakeholders.
- Experience in program coordination, community engagement, and leading initiatives.
- Experience working with diverse populations and vulnerable or underserved communities.
- Knowledge of community-based program delivery.
- Fluency in English is required, French is an asset.
- Requires significant attention to detail and concentration and ability to mentally process large amounts of information.
- The work pace is often unpredictable with significant time pressures.
- Ability to adapt to frequent and often urgent changes in priorities, and make decisions under pressure, work at an unpredictable pace.
- Work requires the ability to work to deadlines.
- Work requires interactions with dissatisfied clients and the resolution of complex client issues.
