Job Summary
The Social Worker III in the Geriatric Division of the Adult Rehab, Geriatric and Palliative Care Program provides specialized professional social work practice across all aspects of the program specializing in the care of the older adult.
Providing intensive therapeutic counselling and support to patients, the Social Worker III utilizes advanced clinical assessment and evaluation, excellent interviewing and listening skills, expert knowledge, and effective negotiation skills in order to provide care to, and advocate on behalf of, patients and their families.
Duties will also include conducting research activities designed to further clinical practice and service delivery; providing expert knowledge and skills within the practice area; developing and implementing a wide range of quality improvement activities; acting as a resource/consultant to a variety of program service areas; and potentially presenting expert testimony in court.
The Social Worker III Provides specialized, advanced, and intensive therapeutic counseling to restore and enhance the bio-psychosocial functioning of individuals, couples and families. Practicing autonomous decision-making in a variety of clinical situations, the social worker III independently screens social work referrals, manages intake and prioritizes waitlists, and is adept at identifying trends and evaluating complex needs.
In addition to the provision of therapy and counselling, the Social Worker III initiates and modifies social work interventions based on treatment progresses, contributing to the overall clinical plan of care for the patient.
The Social Worker III develops and implements quality improvement initiatives such as program evaluation, patient satisfaction surveys, and research activities aimed at program development and improvement.
Other duties include, but are not limited to:
? Maintaining accurate and comprehensive records of assessments, treatment interventions and patient progress.
? Completing legal documents, preparing reports, and providing expert testimony in court as required.
? Preparing reports for program leadership and senior management which evaluates the effectiveness of social work service as well as the program, making recommendation for future directions.
? Provides orientation, training and consultation to other social work staff regarding advanced clinical practice globally in the areas of Social Work practice and, specifically in the area of older adult care.
? Participate in program committees and may chair internal or external committees as required.
? Provides field instruction and supervision to clinical social work students in conjunction with Social Work Schools across Canada

Occasionally there is lifting or physical strength required to carry boxes of resources/materials, flip charts and audio-visual equipment up to 15 lbs.

Visual and auditory concentration is required when reviewing client files/notes and for documenting work into computerized systems.
A higher than normal level of attentiveness and carefulness is required when assessing clients, especially those who may be at a high risk, in order to ensure their health and safety, staff's safety, and to prevent any possible dangers or hazards from happening.
Lack of control over the work pace and time pressures can occur when completing assessments, case documentation and workload reports, managing emergency and routine appointments within the allocated timeframe.
Occasionally there are interruptions due to a crisis situation.
Job Qualifications
Graduation with a Clinical or Integrated Master?s degree in Social Work from an accredited program with the MSW field placement in a clinical practice is required.
Current and continuing registration with the Newfoundland and Labrador Association of Social Workers (NLASW) is required
Minimum of five years of experience within the last seven working as a Social Worker with older adults with acute and/or chronic illness is required.
Experience working with older adults in a community-based setting would be an asset.
Experience with individual, couples, family and group therapy is required.
Knowledge of psychosocial issues related to advancing age, loss of independence, chronic illness including dementia, and the physical, psychosocial, and spiritual impact on the older adults, their family and caregivers.
Knowledge of therapies in stress management, post-traumatic stress disorders, coping and adjustment therapies, suicide intervention and grief and bereavement
Knowledge of relevant legislation, including but not limited to, Mental Health Care and Treatment Act, Adult Protection Act, Medical Assistance in Dying, and the Children, Youth and Families Act.
Knowledge of discharge planning and availability of Eastern Health and community-based resources.
Demonstrated strong clinical leadership skills.
