Reporting to the Regional Director for Primary Health Care and Chronic Disease Prevention and Management, the Collaborative
Practice Manager (CPM) is responsible for the ongoing development and implementation of the Family Care Team in Central
Labrador (Happy Valley -Goose Bay) within the Labrador Grenfell Zone. In partnership with program leadership, the CPM will
provide operational oversight for collaborative practice through effective leadership, change management and team building
for the Family Care Team, targeting key improvement deliverables including access and service delivery, attachment, continuity of care, quality improvement, evaluation, and risk management of primary health care services to support patients and providers within the Family Care Team
Qualifications:
- Post-secondary education in a health-related discipline, system design, project management, or interdisciplinary
studies. (Combinations of education and experience will be considered). - Minimum of five (5) years of experience in Primary Health Care.
- Demonstrated knowledge of the principles of Primary Health Care, Population Health, Health Promotion, the Family
Care Team Health Policy Framework and Health Accord NL. - Knowledge or experience with electronic health solutions (i.e. EMR, electronic health records, virtual health).
- Knowledge and demonstrated experience in change leadership, system transformation and quality improvement
methodology. Institute for Healthcare Improvement (IHI)/ Prosci/ CHA Learning Change Leadership/ Lean or other
change management certification is considered an asset. - Ability to plan, develop, implement, manage, and evaluate program changes, applying quality improvement principles
and processes to change initiatives. - Demonstrated knowledge of and skill implementing change management in a complex environment that is consistent
with the vision, purpose, and operating principles of the Family Care Team Health Policy Framework. - Experience in team development, including gaining trust and cooperation and motivating members of the team
toward achievement of deliverables and goals. - Skills in group facilitation, conflict resolution, and group dynamics.
- Experience working in and understanding of the unique challenges and opportunities of rural health care service
delivery. - Understands systemic racism and applies concepts of cultural safety in engaging with Indigenous peoples.
- Advanced oral and written communication skills, including effective presentation and facilitation skills.
- Work effectively in a computerized environment utilizing standard Microsoft Office applications.
- Physical ability to perform the duties of the position.
Duties and Responsibilities
- Lead the Family Care Team in Central Labrador (Happy Valley-Goose Bay) to meet the key attributes of successful
Primary Health Care as outlined in the Family Care Team Health Policy Framework NL. - Model and promote change leadership in an environment that fosters patient and family centered co-design and
systems thinking. - Collaborate closely with internal and external partners, including other programs and departments, to ensure
coordinated and integrated service delivery. - Promote a culture of physical and psychological safety and learning through team coaching and influence of others to
adopt or consider varying points of view. - Improve access and continuity of care for patients and families.
- Enhance the patient and family experience through the development of efficient, patient centric care pathways
working within a People Centered Care framework. - Facilitate team building strategies to enhance collaboration within the Family Care Team structure.
- Support and mentor team members in the expanded use of the electronic medical record (EMR) to improve access
and quality of care for patients and families. - Facilitate staff development, education, and performance management activities.
- In partnership with team members, develop and measure key performance indicators in the areas of access,
collaboration, Primary Health Care and chronic disease screening, prevention, and management. - Lead quality improvement initiatives based on process and outcome measures.
- Prepare and manage budgets, staffing plans, and resource allocations in collaboration with regional leadership.
- Participate in and support research and evaluation initiatives related to Family Care Teams.
- Participate in the development and implementation of policies, procedures, and protocols that support effective
program operations. - Ensure compliance with regional and provincial policies, standards, and accreditation requirements.
