Title: Transition Home Support Student

Employment Status: Temporary Full-time (2 Months- Summer Student)

In this role you will:
- Provide support to seniors during hospital discharge.
- Support clients during the transition from hospital to home in accordance with established service plan.
- Complete basic errands for service participants as required.
- Conduct basic home management duties.
- Complete risk analysis of home using standardized Risk Assessment tools.
- Facilitate service participant and caregiver engagement.
- Facilitate referrals to Community Support Services.
- Handle and store hazardous products safely.
- Maintain comfort and safely of service participants immediate environment.
- Identify and report any unsafe or hazardous equipment or situations to the Program Manager.
- Take measures to ensure personal safety and to protect the health and safety of service participants.
- Observe and report any concerns related to the service participant and or their environment to the Program Manager.
- Provide companionship to service participants being transitioned home.
- Assist Program Manager with statistics and reports as needed .
What we are looking for:
- Excellent interpersonal skills and the ability to accurately document and record information.
- Strong computer skills, particularly in Microsoft Office applications including Word, Excel, and Outlook.
- Proven organizational abilities with a keen eye for detail and the capacity to manage and interpret complex information.
- Outstanding written and verbal communication skills.
- A proactive mindset with the ability to take initiative and work independently.
- Solid problem-solving skills and the ability to troubleshoot and resolve issues effectively.
Working conditions:
- Be between 16 and 30 years of age at the start of the employment.
- Have a valid Social Insurance Number.
- Valid Driver's license
