About the Role:
A key pillar of Virtual Care, the Remote Patient Monitoring (RPM) program provides chronic disease
patients with an advanced home monitoring program to support self-management and behavioral
modifications to improve quality of care and clinical outcomes, including early detection and intervention of
exacerbation which in turn should reduce ER visits and hospitalizations.
The responsibilities include but are not limited to:
- Provide provincial leadership and accountability for the continued
development, implementation, integration, management, and ongoing evaluation of the RPM program. - Propel the program to meet full benefits realization, acting as the clinical front and engagement ambassador to strategically generate program awareness and achievement of the patient enrollment targets.
- Serve as the main driver of client relationships and communications with key stakeholders.
About You:
Your education and experience:
- A Bachelor’s Degree in a Nursing or Health related discipline required.
- A Master’s Degree preferred.
- A minimum of five (5) years’ clinical experience is required.
- Previous management experience is required.
- Experience using technology to deliver, monitor and advance patient care outside the hospital setting is
required. - Experience in the clinical management of COPD and /or CHF patients in an acute setting is an asset.

to quality and work ethic. - Demonstrated ability to function effectively in a high paced, diverse and changing environment.
- Demonstrated strong interpersonal, facilitation and presentation skills as well as a proven record in
developing collaborative working relations are required. - Knowledge of adult learning principles to apply to a senior population.
