JOB SUMMARY :
The Primary Health Care Access Coordinator (PHCAC) ensures the coordination, efficiency and effectiveness of efforts to improve access to primary health care in the region. As a member of an interdisciplinary team, the PHCAC contributes to the planning, implementation, delivery and evaluation of services, using recognized strategies to achieve results at every stage of service.
Specifically, the PHCAC supports the implementation of interdisciplinary teams and other strategies aimed at providing comprehensive, accessible and coordinated services to a target population. The PHCAC also builds long-term partnerships between various hospital and community stakeholders to meet the needs of the population.
REQUIREMENTS :
- Undergraduate degree in a health discipline
- Additional training in health administration or project management (an asset)
- Minimum of five years' experience in a clinical setting, including two years' experience in project development, implementation or coordination
- Professional experience in the field of primary health care (an asset)
- Experience in change management, quality improvement, and program assessment (an asset)
- Ability to set objectives and commit to achieving results
- Attitude that promotes interdisciplinary communication and conflict resolution
- Ability to prepare and deliver presentations to diverse groups
- Ability to act as a leader and change agent
- Ability to analyze and process indicators, establish a link with evidence based on best practices, and propose recovery plans where necessary
- Ability to use Microsoft Office™ applications (MS Outlook, Word, Excel, PowerPoint, etc.) as well as social media
- Ability to work independently and as part of a team;
- Physical ability to perform the assigned work;
- Good work history (performance and attendance);
- Adherence to professional ethical principles, the Network's management philosophy, and organizational values;
- Compliance with the rules of confidentiality set by Vitalité Health Network.
