Demands, duties, qualifications
About the Role

Reporting to the Regional Manager of Community Supports and Community Health Nursing. The primary purpose of this position is to perform professional community health nursing. The position provides Home and Community Care Nursing care; assists with palliative care program, short term acute program, Personal Care Home Program, along with daily walk-ins, ambulatory care where required. The incumbent must be committed to monitor safety and partner will all team members to improve worker and patient safety by integrating safety into work practices, identifying risks, and implementing improvements.

Qualifications:

? Graduation from an approved/recognized school of nursing program is required;
? Current and continuing practicing licensure with the College of Registered Nurses of Newfoundland and Labrador (CRNNL) is required;
? CNA certification in Community Health is an asset;
? One year of Community Health Nursing (Home and Community Care) experience is preferred;
? Current CPR;
? A valid Newfoundland and Labrador Class 5 Driver license and access to a private vehicle is a condition of employment
? Applicants must also be prepared to demonstrate an ability to be self-directed and exercise independent nursing judgment in a community- based environment with limited supervision; work collaboratively with team members and community;
? Ability to assess community health needs; to develop, implement and evaluate community health nursing programs; to care for clients in ambulatory care;
? Ability to communicate effectively orally and in writing;
? Wound care management course completed or willingness to complete in a time frame identified by the employer;
? The successful applicant will be required to submit a satisfactory Certificate of Conduct and Vulnerable Sector Check;

Duties: (Included but not limited to)

? Works with individuals and colleagues in planning, developing, implementing and evaluating client focused community health nursing;
? Delivery of professional nursing services to clients who are discharged from hospital or referred from the community who require nursing care, this would include wound care, veni-puncture, IV therapy, etc.;
? Referrals for home support services to seniors and long-term care placement;
? Monitors care of clients and reassess appropriateness of care plan through case co-ordination process.
? Co-coordinates the training, supervision, certification and re-certification of delegated nursing procedures to home support workers;
? Assists with Ambulatory care clinics, palliative care programs, Short Term Acute program;
? Liaises with and completing referrals when required.
? Maintains accurate electronic and hard copy records and submits reports and statistics as required;
? Maintains continuing education to remain current in the new methods and procedures;
? Monitors and provides support to Personal Care Homes;
? Completes Wound Care Course and supports standardized wound care initiative;
? Participates in research, committees, program related activities and policy development as required;
? Assists in the orientation of new personnel and acts as a preceptor for students in Community Health, ambulatory care;
? Assists and supports the continuing care programs as required including the palliative care program;
? Works to full scope of practice for Community Health Nursing, personal care homes and ambulatory care;
? Maybe required to do relief;
? Other related duties as required, travel maybe required, may be required to work alone.
