We are searching for a Licensed Practical Nurse to join our Enhanced Care Centre team based in Bridgewater, Nova Scotia.

About the opportunity:
- Performs initial and regular health assessments to maintain patient quality of life.
- Supports development of individualized care plans that focus on the patient's functional abilities and support network.
- Assessing, planning, implementing and evaluating nursing care of patients.
- Ensures compliance with Philosophy of Service and patient care plan to ensure optimal health and wellness for each patient.
- Manages patient medication in accordance with legislative requirements.
- Follows the process in place for safekeeping of medications
- Administers and records prescribed medications in accordance with facility policy.
- Provides consultation regarding patient issues and standards of care to interdisciplinary team.
- Communicates with all other departmental teams to increase awareness of patient health changes to ensure effective service. (e.g. decrease in appetite).
- Ensures all completed care tasks are documented appropriately using identified technology and tools.
- Supports a commitment to ongoing quality improvement, continuous exploration and implementation of new approaches, learning, teaching, and mentoring.
About you:
In addition to placing high value on continuous improvement, collaboration and accountability, you bring:
- A Diploma in Practical Nursing and are currently registered with the Nova Scotia College of Nursing.
- Comprehensive knowledge of nursing and health care practices, excellent communication, and interpersonal skills
- Availability to work a schedule of flexible hours and all shifts inclusive of days, evenings, nights, weekends, and holidays
- Advanced proficiency in the English language is required, proficiency in other languages considered an asset
- Ability to provide a CPR & Emergency First Aid Certificate, Clear Criminal Record Check and Vulnerable Sector Screening upon hire
- A passion for the healthcare sector and/or ensuring seniors have access to quality accommodations, services and care.
- Asset: Previous experience supporting admission, discharge, and care of vulnerable populations with complex social situations; implementing care plans and identifying resources that will give the patient every opportunity to be successful in returning to home/community.
