Home First Project Manager/Clinical Mentor
Temporary Full Time (until March 2024)
Location: To Be Determined

Reporting directly to the Regional Director of Rehabilitation and Community Support Services, the Home First Manager/Clinical Mentor will work in partnership and support the Clinical Nurse Manager of Community Health Nursing and Community Support Manager to set the direction for Home First, Community Supports and Community Health Nursing. This position will also provide professional and clinical mentorship within the Community Supports Program. This leadership role will be instrumental in implementing and facilitating intensive case management of complex situations with Labrador-Grenfell Zone across the continuum of care (acute, long-term care and community). This position will oversee the provision of services for all clients under the Home First Initiative and will ensure region-wide continuation, coordination, development, implementation and standardization of professional practice standards.

Qualifications:

" Undergraduate degree in a health-related field (i.e. Nursing, Social Work, Physiotherapy or Occupational Therapy)
" Current and in good standing with professional licensing body;
" Management experience within the last four years in a complex health care setting;
" Two years' experience within the last five years working in Community Supports / Community Health;
" Ability to work independently with limited supervision;
" Experience with program development and implementation is an asset;
" Familiarity with the provincial home support and long-term care operational standards;
" Experience in data collection and interpretation;
" Good written and oral communication skills;
" Driver's License;
" Willingness to travel within the Zone;
" The successful applicant will be required to submit a satisfactory certificate of conduct.

Duties: (Include but not limited to)

" Leadership for the Home First Network;
" Provide education to staff on Home First processes;
" Implement case management and support plan framework, and documentation requirements for Community Supports Program and Community Health Nursing;
" Ensures person and family-centered care that wraps available resources around the client;
" Develop key performance indicators, benchmarks and scorecards;
" Monitor client access to services and wait times for services;
" Develop and implement workload tools;
" Develop and implement strategies to improve client outcomes;
" Act as a resource and provide clinical consultation and advice, direction and assistance to Social Work and Nursing Clinical Coordinators/Consultants;
" Provide educational opportunities for clinical coordinators on mentoring/coaching activities to support clinical staff development;
" Support clinical coordinators in their professional development;
" Assess clinical competence and implement strategies to promote clinical excellence;
" Acts as a resource on clinical specialty issues to clinical coordinators and regional leadership;
" Develop workshops to improve clinical assessment skills and evidence-informed practice;
" Coordinate educational opportunities and provide mentorship to the Home First Initiative; and
" Coordinate various change management activities.
" Travel within the region;
" Performs other related duties as required;.

Salary:

" Salary is in accordance with the Hay Management Pay Scales (HL22), plus the applicable Labrador Allowance.

Hours of Work:

" 70 hours Bi-weekly; variable shifts.
