The incumbent will collaborate with the Project Manager and the Community Coordinators, the Community Connector will work one on one with seniors in person to help identify their needs as part of the Connection NB Projec and work with them to get connected to resources and services in the community. This pilot project is aimed at reducing social isolation and improving well-being in older adults in various communities in New Brunswick. In this role, you will:
- Visit with participants within the community to administer surveys and questionnaires.
- Engage participants in conversations that may highlight any challenges or issues in their life.
- Assist participants in personal goal exploration and identification.
- Link participants with volunteer organizations or community-based organizations.
- Assist in accompanying participants to resources/services, as needed.
- Promote local community events, programs, services, and activities to the participant and encourage their participation.
- Work collaboratively with a multidisciplinary team, including CRC, community medical and social support services to identify and/or create 'out of the box' solutions to address participants needs.
- Engage with organizations to increase number of partnerships, awareness of programs and improve depth of existing ones.
- Complete status reports and provide updated logs to the Community Coordinators and Project Manager
What we are looking for:
- A post-secondary education in a related field plus 1-2 years of job-related experience or an equivalent combination of education and experience.
- Education designation that may be compatible with this role may include: Personal Support Work, Recreational Therapy, Human Services, Gerontology, Social Work, Occupational Therapy, Nursing and other related designations.
- Experience advocating and mobilizing services for clients, using a person-centered approach.
- Experience working with seniors is an asset.
- Strong knowledge of community resources, health and social system in New Brunswick.
- Strong communication skills, both written and verbal. Fluent in English, French is an asset.
- Ability to work independently with minimal supervision.
- Ability to explore participants needs and provide written reports of visits.
- Strong interpersonal skills and active listening skills.
- Valid driver's license with willingness to travel frequently within specified areas. Ability to provide a satisfactory driver's abstract. Access to a vehicle is mandatory.
Working conditions:
- This is a full-time position until 31-Mar-2024.
- Hybrid position with combination of home-based, in-office and community-based work.
- Travel to other locations in New Brunswick is expected.
