The Nunee Health Board Society (NHBS) is seeking a Homecare and Community Team Lead to join our Community Health Team, this is a Permanent Full Time in Fort Chipewyan, Alberta.

The incumbent will combine health sciences, knowledge and experience in assessment, supervisory and clinical nursing skills to effectively coordinate and deliver client care to individuals and families needing acute care, chronic, or palliative care approached.

Promote wellness through an integrated team approach at the Nunee Health Board Society and with various health partners. The health team at Nunee includes Nurse in Charge, Treatment Nurses, Licensed Practical Nurse, Home Care & Chronic Nurse, Community Health Representatives, Personal Support workers, NNADAP workers, Maternal Child Health worker, and a Health Promoter.

This position offers you the opportunity to:

  • Deliver direct nursing care as required
  • Using structured process, in collaboration with the client and family, assesses, plans, delivers, evaluates, and documents and individualized client assessment which reflects the client goals and priorities. Provides consistent follow-up and changes plans of care as needed.
  • Promotes and provides health related knowledge and treatment services to clients and caregivers, supporting independence and family and community values.
  • Document assessment, care plans, reassessments, and service delivery on clients personal Home Care Health Record.
  • Provides supervision, education and support to other nurses, care aides and homemakers on the homecare team.
  • Develops, facilitates, implements, and modify health promotion activities based on the needs of the client
  • Develops nursing care and teaching strategies to assist/families dealing with chronic illness. Participate and attend education in-service or provide other educational programs to health care staff as required.
  • Collaborate with members of the interdisciplinary team to ensure effective, consistent, and comprehensive care planning and delivery.
  • Maintain a safe environment for clients/family unit and staff
  • Other duties as required.

Qualifications:

  • Baccalaureate Degree in Nursing from a recognized program.
  • Minimum of two (2) years' experience in Home Care.
  • Current licensed/registered in Alberta (CARNA).
  • ACLS, PALS, BCLS and Immunization certification.
  • Applies knowledge, skills, judgment and competencies required for safe, independent and inter-disciplinary practice by meeting nursing practice standards as outlined by CARNA and related professional associations.
  • You will possess excellent interpersonal communication and organizational skills, proficiency with computers, and be medically fit.
  • Maintain accurate records, document and reports. Previous experience with electronic charting systems (EMR) an asset.
  • Applies NHBS values and professional practice standards to patient/client care and service.
  • Current Criminal Records Check and Valid Driver's license.
