Reports to: Resident LPN Manager
This is a Temporary Part-Time Position, 0.56 FTE
JOB SUMMARY:

The Licensed Practical Nurse is an integrated member of the interdisciplinary care team and is responsible for providing direct Resident care within the scope of practice as defined by the College of Licensed Practical Nurses of Alberta (CLPNA), the Continuing Competency Program (CCP), and as noted in the Collaborative Care Model. The LPN participates in the assessment, planning, implementation and evaluation of care and adheres to the Park Place Seniors Living Policy and Procedure Manual. The Licensed Practical Nurse functions independently and works under the guidance of the Resident Care Manager to further provide leadership, support and direction to Mental Health Aides on an ongoing basis. Responsible for current and valid registration.
TYPICAL DUTIES AND RESPONSIBILITIES:
  • Provides ongoing monitoring of Resident response to interventions identifies deviations from expected outcomes and responds accordingly
  • Collaborates in Resident admission and discharge
  • Provides information and advice to Residents, Families and Staff as needed
  • Report significant changes in Residents status and concerns pertaining to team functioning to RCM immediately
  • Informs the Resident Care Manager of situations that are beyond the scope of ability or experience of the LPN
Planning
  • Develops and prioritizes, reviews an individualized plan for each Resident with the Interdisciplinary Team for the physical, psychological and social care
  • Assists the physician with examination, diagnostic tests and therapeutic measures
  • Implements prescribed interventions (medications, medical treatments and procedures) and performs independent nursing interventions
  • Ensures the ongoing administration of the Resident Care Plans and reviews this with the Interdisciplinary Team regularly
Evaluation
  • Evaluates the effectiveness of Nursing Care treatments and informs the Resident's Physician of significant changes in condition of the Resident
  • Works with the Interdisciplinary Team to problem-solve when required
  • Evaluation of Care delivery
Documentation
  • Participates by communicating and observing the Resident, communicating with direct Care Staff, interdisciplinary health professionals, and family
  • The LPN is responsible for ensuring: care plans are carried out, documenting in Progress Notes, assessments, profiles, tracking and electronic records, within the LPN scope of practice
  • Records complete and accurate relevant data for every Resident according to the Care Centre's Charting protocol
  • Follows and assists in revision of facility policies and standards of Nursing practice
  • Observes Staff performance and participates in performance appraisals of MHA Staff
Administrative and Operational Support
  • Suggests Resident Care service enhancements to the Resident Care Manager or Director of Care
  • Corrects unsafe conditions and practices within the Nursing Department.
  • Routine Audits
  • Active input into nursing committees and follows through on committee recommendations
  • Attends monthly Staff meetings
  • Assists with the enforcement of fire and safety regulations
  • Participates in special programs as assigned (i.e. Flu immunization)
  • Arranges for Nursing Staff replacement after hours and weekends
  • Consults with other professional team members as indicated (i.e. PT, SW, RT, and OT)
  • Assists with preparing Doctor's list as required
  • Coordinating team assignments daily
Professional Responsibilities
  • Serves as an advocate to ensure that the Resident's needs are met
  • Participates in research, in-service classes' onsite and off, Staff development and orientation when required
  • Adherence to Professional and Care Centre protocols, practices and standards
  • To ensure that their practice meets the legislative requirements and College of Licensed Practical Nurses of Alberta Standards of Practice

QUALIFICATIONS:
Education and Training: Graduate from an accredited School of Licensed Practical Nurses. Current CPR and First Aid is required.
Professional Designation and Affiliations: Active registration with the College of Licensed Practical Nurses of Alberta.
Physical Demands: The LPN position requires a significant amount of lifting, walking, reaching, bending, pushing, standing and sitting. In an average day the employee spends 2 to 6 hours standing or walking. Lifts a maximum of 50 lbs., frequent lifting and/or carrying of 20 lbs. Must be able to manage stressful work situations with dexterity.

High Risk Element: Ergonomics, slips/trips/falls and psychological related to providing care to Residents.
Prior Experience:
A minimum of five years' experience as a LPN is preferred.
Previous experience in Mental Health, Geriatrics or Long-Term Care preferred.
Equipment Used: Computer, photocopier and facsimile.
Various mechanical aids (transfer belt, lifts) for Resident Care
