About Us

The Prince Edward Family Health Team (PEFHT), comprised of a multidisciplinary health care team, provides integrated, patient-centred primary care through direct office care and innovative, award-winning programs and services aimed at meeting the distinct needs of the Prince Edward County community.

The Role

A challenging and rewarding role, the NP will engage in autonomous decision making and authority within scope of practice, as well as shared decision-making outside the NP scope. Clients will include individuals, families, and the community as a whole, with a focus on caring for our most fragile, often including the 65+ population, in their homes. This role will include episodic primary care activities in clinic for same-day access, and working closely with other nursing professionals and community partners to streamline care that optimizes keeping people in their homes. There may also be opportunities to support palliative care.

The PEFHT is a strong supporter of our community, and the NP may be called upon to shift to different functions, depending on community needs and the context of Covid-19 recovery such as working collaboratively with our physicians in our local hospital for inpatient care, or providing some support during peak volume times in the emergency room. This role requires someone who is flexible and willing to apply their keen skills as needed, and be able to adapt quickly to new situations. A sense of humour and collegiality is a must.

Must-Haves

  • Registration in the Extended Class with the CNO; member of RNAO or NPAO; with liability insurance.
  • 5+ years clinical nursing experience.
  • Superior skills in assessment and diagnosis.
  • Excellent verbal and written communication, facilitation, and interpersonal skills; ability to build and nurture positive working relationships both within and outside of the PEFHT.
  • Knowledge of health care facilities and community health services, social agencies, etc.
  • Demonstrated ability to work independently as well as collaboratively within a multidisciplinary team, and within a brisk work setting.
  • Cooperative, team-based approach balanced with a high degree of self-motivation, accountability, and leadership.
  • Proficient with electronic medical records and standard office software.
  • Valid driver's license, access to a reliable vehicle, willingness to provide home visits
  • Willingness to obtain a Vulnerable Sector Police Check.
  • Proof of having received primary series of COVID vaccines.

    • Not have been employed as a nurse in Ontario in the six months prior to being hired;
    • Commit to 24 months of employment with the PEFHT at a full-time work schedule; and
    • Not simultaneously receive funds from the Ministry of Health for the Tuition Support Program for Nurses.
