ClusterOutreachSupportMentorJob Summary:

As aClusterOutreachSupportMentor, you willprovideindividualized supporttoindividuals bothwithinthe cluster locationand in the community, promoting independence, safety, and meaningful engagement in daily life.Youwill work closely with multiple individuals throughouttheshifts,assistingwithoccasionalpersonal care, life skills, emotional wellness, and community access.

This particular opportunity is tosupport individualsresiding in8 suites within a 77-suite building.This is a unique opportunity for professionals who excel at building relationships, taking initiative, and working independently whilemaintainingstrong professional standards.

The ideal candidate would have Crisis Intervention experience, workwellunder pressure,havestrongboundariesand theabilitytode-escalate conflicts. Experience with mental health is an asset.

Preference may be given to candidates with experience or training in:

  • Non-Violent Crisis Intervention (NVCI)
  • Security experience
  • Shelter experience
  • Naloxone training
  • Suicide Prevention / ASIST training

Available Shifts:

We are currently hiring for the following schedules, with coverage required 7 days per week:

  • 12:00 PM 6:00 PM

Key Responsibilities

  • Provide one-on-one and cluster-based support to individuals inclusterhomeand community settings
  • Support individuals withmeal planning, grocery shopping, cooking, and household upkeep
  • Administer and documentmedication regimensaccording to protocol
  • Attend and advocate duringmedical appointments
  • Provideemotional support, companionship, and crisis intervention
  • Assistwithlife skills development, including budgeting, bill payments, transportation, and self-care
  • Support individuals in accessingemployment, volunteer roles, programming, and recreational opportunities
  • Facilitate connectionstocommunity resourcessuch as food banks, clothing resources, and support agencies
  • Encourage social inclusion and participation in community activities
  • Maintainclear,accurate, andtimelydocumentation
  • Build strong, professional relationships whilemaintaininghealthy boundaries
  • Work independently and collaboratively with team members to meet support goals

Qualifications

  • High School Diploma(minimum)
  • 12 years of experience working with adults with intellectual disabilities, transferable life skills, and relevant lived or professional experience will also be considered
  • Some post-secondary courses in the social services field (preferred)
  • Knowledge of disabilities, disorders, and mental health issues
  • Strong communicationskills
  • Advocacy,flexibility, andtheability to adapt to changing support needs
  • Comfortable working alone

Required Certifications & Requirements

  • Current First Aid & CPR Certification
  • Valid Manitoba Drivers License
  • Access to a safe vehicle for work purposes
  • Criminal Record Check with Vulnerable Sector Search
  • Adult Abuse Registry Check
  • Child Abuse Registry Check
