What the HART Hub Entails

The Somerset West Community Health Centre (SWCHC) is excited to announce the launch of the 24/7 Homelessness and Addiction Recovery Treatment (HART) HUB, a transformative initiative designed to provide integrated support to people experiencing complex health and social issues related to substance use and addictions: and aims to connect people who are experiencing homelessness or are at risk of losing their housing and/or people who have mental health, to treatment and preventative services, while addressing basic needs such as food, clothing and showers.

What Youll Do

Reporting to the Manager, HART HUB, the Housing Case Manager works as part of an interdisciplinary team to organize, and HART HUB clients find housing and connect with appropriate services.

The Housing Case Manager will develop care plans for clients with substance use disorder, homelessness, and need mental health supports. This is a highly independent position that requires meeting clients at the HUB and in the community. This will include helping clients secure financial aid, employment services, help with housing search, access to basic needs such as: food, hygiene products, clothing, furniture, and other supports as needed. The Housing Case Manager will help with viewing units, assisting with lease-signing, review lease and rules to meet clients where they are at. The Housing Case Manager will also collaborate closely with members of the HART HUB team such as Primary Care, Peer Coaches, System Navigators, recovery & treatment and counselling services to access wraparound services.

What Were Looking For

  • Life and Work Experience
  • Two plus years experience and passion for working with high-risk populations and active drug users OR equivalent combination of education and relevant experience
  • Lived experience of drug use and/or experience in advocacy for persons who use drugs considered assets
  • Experience in an outreach, drop-in or shelter setting is an asset
  • Your Education Path
  • Degree in Social Work (BSW); Social Service Work; Social Sciences, or equivalent educational background in a related discipline
  • Required Language
  • Fluency in English required. Working knowledge of French an asset.

  • Knowledge of the impact of social/cultural/economic factors on the determinants of health
  • Experience supporting people living with homelessness, mental health and addictions challenges
  • Knowledge of housing programs and rooming houses in the Ottawa area
  • Knowledge working with marginalized populations
  • Experience in a drop-in, outreach or shelter setting is an asset
  • Experience in overdose response is an asset
  • Ability to use capacity-building and skills-building approach in working with clients
  • Ability to work flexible hours based on client need
  • Proficiency in the use of computers and various software applications
  • Strong organizational skills
  • Excellent communication/interpersonal skills
  • Enhanced skills in non-violent crisis intervention and de-escalation
  • Knowledge of best practice guidelines for individuals with substance use, and suicide assessment.
  • Effective organizational, critical thinking, problem solving and independent decision-making skills
  • Demonstrate and advocate for welcoming practices that are anchored in trauma-informed practices
  • Demonstrated ability and clinical expertise working with clients in crisis
  • Ability to work independently, set priorities and manage time effectively
  • Ability to work collaboratively in an interdisciplinary team
  • Ability to work with people with concurrent disorders
  • Ability to work under stress with multiple demands
  • Valid CPR and First Aid
  • Successful completion of police record check for vulnerable populations
  • Commitment to working from a health equity and anti-oppressive lens
  • Valid drivers' license, insurance and use of a personally owned vehicle

Your Day-to-Day at a Glance

The Housing Case Manager will have key responsibilities in:

  • Active screening of clients seeking services
  • Conduct needs assessment and coordinate appropriate housing placement
  • Support with house viewings
  • Assisting clients in obtaining resources for healthier living (i.e., personal care items, income, furniture, etc.)
  • Review leasing documents, tenant rules and regulations
  • Performs health promotion activities including referring clients to primary care and other outreach services
  • Provides Crisis Intervention services
  • Provides administrative supports, i.e: completion of forms
  • Provides information on community resources and social services systems/ and agencies and refers accordingly
  • Establish and maintain collaborative linkages between HART HUB and outreach services to form wraparound supports for clients
  • Co-ordinate case conferences with all service providers who work with the client
  • Assist clients to identify, build and maintain a natural support system of friends, family and significant others
  • Teach skills that will enable clients to maintain their housing and live in the community
  • Support clients while they are in hospital or jail
  • Assist clients in identifying and advocating for their civil and legal rights
  • Other duties as required

Provides Outreach Services

  • Works in collaboration with Outreach NP to provide services and support to clients living in rooming and supportive housing
  • Provides appointment accompaniment when deemed necessary/appropriate
  • Provides home visits to assess and provide support to clients

Provides Advocacy

  • Advocates for clients' needs
  • Advocates with community on broader issues regarding clients
  • Advocates and provides information within the centre by identifying clients/program issues
  • Advocates for clients to prevent evictions
  • Prioritizes issues and mobilizes resources to meet client needs
