Status: Full-Time, Fixed Term (12 months)
Schedule: Primarily MondayFriday, 8:00 AM 4:00 PM, with flexibility for occasional evenings
Reports to: Manager, Community and Justice

We believe every young person deserves a fair start, a supportive network, and the tools to thrive. Our Youth in Transition Program, funded by the Ministry of Children, Community, and Social Services, helps youth aged 1523 move from CAS care or supports into independent living.

We work alongside youth to secure housing, build life skills, pursue education or employment, and strengthen their connections to community supports. Our approach is youth-centered, trauma-informed, and focused on removing barriers so young people can step confidently into their futures.

About the Role

As our Youth in Transition Worker, youll walk alongside youth during one of the most important times in their lives the move from care to independence. Youll be their advocate, guide, and connector to resources, ensuring they have what they need to succeed.

This role is hands-on and flexible, requiring a balance of relationship building, practical support, and resource navigation. Some days youll be meeting youth in the community, others youll be connecting them to services, and sometimes youll just be there to listen and help them plan their next step.

What Youll Do

Direct Support

  • Build trust and rapport with youth using a flexible, youth-driven approach.
  • Help youth identify their goals and develop action plans.
  • Support access to housing, education, employment, and community services.
  • Provide transportation and accompany youth to important appointments.
  • Offer crisis intervention, conflict resolution, and emotional support.
  • Teach and model life skills, problem-solving, and healthy relationship practices.

Case Management & Documentation

  • Complete assessments and develop individualized support plans.
  • Maintain accurate case notes and program reports.
  • Collaborate with CAS staff, community agencies, and other supports.
  • Manage a caseload of approximately 20 clients annually (FTE).

Community Engagement & Resource Development

  • Build strong partnerships with community organizations.
  • Promote the YIT program to service providers and stakeholders.
  • Maintain up-to-date resource materials and referral lists.
  • Represent the agency at community meetings and youth-focused events.

What You Bring

  • Diploma or degree in Social Services, Child and Youth Work, Social Work, or related field.
  • Experience working with at-risk youth.
  • Strong ability to engage youth quickly and work from trauma-informed, anti-oppressive, and culturally inclusive frameworks.
  • Skilled in de-escalation, active listening, and crisis response.
  • Ability to adapt schedules, communication, and approaches to meet youth needs.
  • Understanding of the child welfare system and local community resources.
  • Proficiency with Microsoft Office and data management systems.
  • Valid G drivers license, access to a reliable vehicle, and $2 million liability insurance.
  • Valid First Aid/CPR, Crisis Intervention, and ASIST training (assets).

Working Conditions

  • Combination of office and community-based work.
  • Standard hours with occasional evening flexibility.
  • Regular interaction with youth, CAS workers, community partners, and agency staff.
  • Manual dexterity required for computer and office equipment use.
