The Nurse Practitioner (NP) plays a critical role in providing comprehensive primary healthcare services, with a specialized focus on refugee health and pediatric care. The Nurse Practitioner will maintain and manage their own roster of patients, working within a multidisciplinary team to deliver high-quality, client-centered care. This role requires a commitment to providing accessible healthcare services and working collaboratively to improve health outcomes for vulnerable populations.Main Responsibilities/Duties:
- Provide comprehensive primary healthcare services to individuals and families, with a focus on refugee and pediatric populations.
- Conduct health assessments, diagnose conditions, and develop treatment plans in accordance with best practices and clinical guidelines.
- Prescribe medications and therapeutic interventions within the NP scope of practice.
- Manage and coordinate care for patients with chronic and complex health conditions.
- Collaborate with an inter-professional healthcare team, including physicians, nurses, social workers, and community health professionals.
- Provide patient education, counseling, and advocacy to promote health and well-being.
- Support the development and implementation of health programs tailored to the needs of refugee and pediatric patients.
- Maintain accurate and timely electronic medical records (EMR) and documentation.
- Participate in quality improvement initiatives and professional development activities.
- Support and contribute to our company's commitment to workplace equity, diversity, and inclusion.
- Successful completion of a post-graduate Primary Care Nurse Practitioner program.
- Current Extended Class Registration with the College of Nurses of Ontario.
- Proof of current membership or alternative professional liability coverage.
- Current CPR certification (Level C or HCP).
- Demonstrated experience in community and/or primary care.
- Experience in chronic disease management and prevention.
- Strong ability to work both independently and collaboratively within a healthcare team.
- Excellent oral and written communication skills in a clinical setting.
- Proficiency in the use of electronic medical records (EMR); Accuro experience is an asset.
- Passion and experience in Refugee Health and Pediatric Care are considered strong assets.
- Employment Type: Permanent, Full-Time (37.5 hours per week) Part-time arrangements will be considered.
- Salary: $52.10 - $62.76 per hour + on-call stipend
