We are seeking a dynamic, highly motivated Intensive Case Manager who demonstrates our values of compassion, accountability, responsiveness, and empowerment to our clients, families, and each other.
TheIntensive Case Manager covers off staffing needs within our Wesley Supportive Housing Program.WSH (Dual Diagnosis) is a housing program for 10 adults living with a Dual Diagnosis. This program in in partnership with the YWCA and St Joseph's Hospital. This position will take a lead role in coordinating case management activities for Wesley Supportive Housing Clients.
We are looking for a highly motivated staff who will use their skills to provide flexible intervention, assist clients to work on individual goals, assist with life skills development, support clients with programs and provide support and understanding for the struggles they may encounter.
This position is a unionized, full-time, temporary position. The contract is until December 31, 2025. The rate of pay is $24.38 per hour. Hours per week are 40.
Key Responsibilities:
- Provide support to individuals with complex special needs, behavioral and mental health challenges in a residential setting
- Ensure all resident files including individual care plans, contact information, community support plan, health records, safety plans and changes to support are current.
- Coordinate appointments, referrals and set staff reminders for all Wesley Supportive Housing residents
- Initiate and coordinate case conferences when required
- Communicate any changes in care or information to all staff
- Create and maintain de-escalation plans for all residents
- Follow the behavior support plans, daily activity schedules, safety plans, individual case plans and other programs or activities available within the respective programs.
- Offer crisis intervention as appropriate; using safety plans, care plans, emergency intervention and on call as appropriate.
- Respond appropriately in emergency situations using conflict resolution techniques, motivational interviewing and sometimes just an open mind and willingness to listen.
- Work effectively within a strengths-based personcentered approach to service delivery.
- Administer medications and appropriately document distribution, according to the established guidelines of the program, as applicable.
- Document all individual client interactions, participation in groups or activities as well as a brief shift change summary articulating the client's needs, behaviours and other pertinent information to be shared.
What you will bring:
- Post-secondary degree/diploma in Social Services.
- Registration and member in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) is required
- Preference will be given to staff currently working in Supportive Housing
- Experience providing client-centered services within a multi-disciplinary team of providers.
- Excellent communication and documentation skills.
- Must be willing to work flexible hours.
- Non-violent crisis intervention - first aid/CPR certification.
