Overview
The Case Manager II delivers intensive, client-centered case management to Guests with complex needs, including those experiencing homelessness, substance use, and mental health challenges. Using a trauma-informed, harm reduction approach, the Case Manager II coordinates care and supports through collaboration with internal teams and external partners to promote housing stability, health, and well-being.
What Youll Do
- Engage Guests with high-acuity needs using a respectful, trauma-informed approach.
- Provide tailored, housing-focused case management that supports autonomy and well-being.
- Coordinate services across internal/external teams and systems for continuity of care.
- Build trust-based relationships with Guests while maintaining ethical boundaries.
- Support Guests in establishing goals related to housing, health, and life skills.
- Maintain accurate, timely documentation in the electronic case management system.
- Participate in team meetings and collaborative care planning.
- Organize case consultations and coordinate with external partners.
- Support crisis management through safety planning and de-escalation.
- Supervise BSW student placements as needed.
- Uphold policies on Guest/workplace safety and contribute to quality improvement.
- Perform additional duties as required.
What You Bring
- Degree in Social Work required; registration with NLASW
- At least 2 years' experience in case management or a related role with marginalized populations.
- Knowledge of local/provincial services and housing resources.
- Strong interpersonal skills and understanding of systemic barriers.
- Skilled in service navigation, planning, and conflict resolution.
- Proficient in documentation and professional communication.
- Adaptable, creative problem solver in dynamic situations.
- Clear Record of Conduct and Vulnerable Sector Check required.
Equivalencies in education and experience will be considered.
