COMPENSATION: $55,239 - 67,560 annually prorated

To help fulfill its mission, LOFT is hiring a Full-time Permanent Case Manager, Psychogeriatric.
Job Summary
As a Case Manager, Psychogeriatric you will be part of a Mobile Support Team that provides direct care, education and support to older adults and their care partners, who are demonstrating responsive behaviors that may be associated with various types of dementia, mental health, substance use or other neurological conditions living within the community within a specified geographical boundary. You will complete a comprehensive behavioral assessment and an evidence-based Behavioral Support Plan. You will assist in the creation and coordination of a support network through establishing linkages with other community agencies and practitioners to coordinate care for the clients. In addition, you will provide
direct service to support the client, family members and/or care-givers to mitigate escalating responsive behaviors. You will also liaise, collaborate and communicate with all members of the Behavioral Support Services team and other partners within the circle of care.
The Behavioural Support Services - Mobile Support Team provides direct care, education and support to older adults and their care partners, who are demonstrating responsive behaviours/personal expressions that may be associated with various types of dementia, mental health, substance use or other neurological conditions living within the community within North Toronto, York Region and South Simcoe. The team completes a comprehensive behaviour support assessment and creates a person-centered Behavioural Support Plan. Transition support is provided to those individuals who a transitioning to another care destination or to another care team. Serving over 900 individuals per year, the Behaviour Support Service Mobile Support Team delivers service in the individuals own home and community.
What You Will Do
  • Development and implementation of a Behavioural Support Plan tailored to the needs of the person served designed to improve quality of life and assist in the mitigation of responsive behaviours. Identify and refer to resources specific to those needs and issues;
  • Act as a liaison and advocate for the person served among those involved with the individual's care including family, other care-givers, medical and other multi-disciplinary providers;
  • Complete Behaviour Support Assessment (P.I.E.C.E.S. format) and provide timely and accurate record keeping and reports as required;
  • Provide support, on-going training and consultation to colleagues, family members and/or other care-givers to assist in mitigating escalating responsive behaviours;
  • Provide supportive counselling or referral and linkages to health care, programs or service providers;
  • Provide crisis management/intervention and assist in the development of crisis plans for persons served;
  • Develop respectful and trusting relationships with persons served using Recovery and Psychosocial Rehabilitation principles;
  • Maintain strict confidentiality and follow all other required policies and procedures;
  • Consistently interact with persons served, staff, family members, other community agencies and partners in a professional manner that reflects favourably on the organization and the Behavioural Supports Ontario Project;
  • Effectively participate as a member of the Mobile Support Team; attend staff/team meetings, care conferences for the person served, other multi-disciplinary meetings, committee meetings etc as required;
  • Attend and actively participate in all required training;
  • Liaise, collaborate and coordinate with other professionals, service providers and partners within the Behavioural Support Services;
  • Complete other duties, from time to time, as assigned by the Program Manager.
What You Bring
Must Haves
  • Completion of a diploma/degree in Social Services/Human Services or equivalent;
  • Minimum three years related experience in providing support to vulnerable seniors and older adults with responsive behaviours related to mental illness, addictions, dementia or other neurological conditions;
  • An understanding of harm reduction, recovery and psychosocial rehabilitation principles, the therapeutic relationship, and an ability to come to an agreement with persons served on goals/priorities;
  • Crisis intervention skills, with well-developed problem solving abilities and interpersonal skills;
  • Training/knowledge of P.I.E.C.E.S., U-First approaches or willingness to receive training;
  • Ability to collaborate effectively with other members of inter-professional care teams, persons served, family members and community team members;
  • Good oral and written communication skills and computer literacy, including MS Word and Excel;
  • Counselling and responding skills;
  • Creativity and flexibility to approaches;
  • Knowledge of community resources and supports;
  • Ability to work effectively as part of a team and interact with workers from a variety of agencies;
  • Strong assessment skills;
  • Understanding of cultural competency and framework
  • Valid drivers' license and access to own vehicle is required;
Nice-to- Haves
  • Physically able to perform demanding body mechanics (i.e., lifting, transferring, etc.)
  • Demonstrated effectiveness working with minimal supervision with proven reliability and trustworthiness
  • Ability to communicate in a different language, an asset.
Work Environment:
Staff will be required to work in client's own homes and may therefore be exposed to secondhand smoke. Occasional evening and weekend hours may be required. Frequent travel will be required using own vehicle.
