The North Lambton Community Health Centre is a community governed health centre serving identified priority populations in a defined catchment area. The Centre operates multiple sites in Ontario: Forest, Kettle & Stony Point, Watford, and Sarnia. Our interdisciplinary team provides primary health care, health promotion and community development services.
The North Lambton Community Health Centre, in partnership with the Canadian Mental Health Association, is currently seeking applicants for the following position:
1.0 FTE NURSE PRACTITIONER
HART Hub (37.5 hours/week)
The Nurse Practitioner (NP) will play a pivotal role in delivering comprehensive, client-centered care to individuals experiencing homelessness and concurrent mental health and addiction challenges. Working within a multidisciplinary team, the NP will provide primary and episodic care, mental health and addiction support, and facilitate access to housing and social services. This role is integral to the Ontario government's initiative to establish Homelessness and Addiction Recovery Treatment (HART) Hubs, aiming to enhance service access and coordination for vulnerable populations.
Qualifications:
Undergraduate degree in nursing from a recognized university (or equivalent combination of education and experience).
Registration in the Extended Class with the College of Nurses of Ontario.
Three to five years nursing experience in a community setting or combination of community and hospital or public health settings; experience in a primary care setting, an asset.
Proficiency in the use of computers and various software applications.
Participates in CNO Quality Assurance Program.
Has and maintains CNO Professional Liability Insurance.
Experience with marginalized populations an asset.
This is a permanent position working 37.5 hours per week, Monday to Friday. The rate of pay for this 1.0 FTE position is on our newly revised six-step grid. The rate will be based on experience and education. The incumbent will work primarily at the HART Hub location in Sarnia.
