Position Title: Multicultural Health Navigator for Somali and FrenchCommunities

Classification: Community Health Worker Level 1

Job Type: TERM 18 months, 1 FTE (35 hrs/ week)

Department: Primary Health Care

Reporting to: Manager, Ottawa Newcomer Health Centre

Compensation : $28.78 -$34.72/hr plus a comprehensive compensation package that includes time-off entitlements, health benefits, life insurance, and participation in the HOOPP pension plan.

What the ONHC Entails

The Ottawa Newcomer Health Centre (ONHC) is a proud program of the Somerset West Community Health Centre that provide healthcare, offers timely, coordinated and culturally appropriate health and social services to newcomers. Our approach to care is through a health equity lens with a focus on addressing the social determinants of health to improve the lives and wellbeing of our clients.

What Youll Do

The multicultural health navigator assists newcomer, immigrant, and refugee clients
to navigate the healthcare system. The incumbent works closely with other members of the inter-professional team to provide health education, advocacy, and practical assistance while playing an intermediary role between clients and service providers within the community. The ideal candidate will understand and support Somerset West Community Health Centres mission, possess excellent interpersonal skills, community liaison ties, organizational capabilities, and reporting abilities. The individual is motivated and passionate about working with newcomers providing case management support and enjoys working in a fast-paced environment.

What Were Looking For

• Your Education Path
• Degree or diploma in social sciences or equivalent combination of training and experience
• Degree or diploma in a health discipline or equivalent, an asset

• Life and Work Experience
• Minimum of 2 years experience in a community setting with emphasis on case management, community engagement, practical assistance, and case coordination support for clients
• Minimum 2 to 3 years experience working with diverse client populations, specifically with newcomers, refugees, and immigrants with a focus on Somali and French communities.

• Ability to liaise with external parties and to advocate for clients and provide home visits as necessary.
• Commitment to strength-based philosophy and practice
• Assessment and intervention skills; sound judgment
• Ability to provide basic education to individuals and families.
• Excellent communication skills, able to bridge gap between clients and service providers/service agencies and advocate for clients.
• Strong interpersonal and organizational skills
• Ability to work independently and liaise with internal and external partners.
• Must be able to work on multiple tasks simultaneously to meet
• Respect the privacy of all clients and adhere to strict confidentiality protocols to protect client personal and sensitive health and social information.

Your Day-To-Day at a Glance

• Provide health navigation: Assist clients with navigating healthcare services, including scheduling appointments, understanding medical instructions, and accessing appropriate healthcare resources.
• Health education: Deliver health education sessions based on ONHC educational modules that are tailored to the needs of our newcomer communities.
• Case management: Conduct intake assessments, convene home visits, create individualized support plans, follow up on client progress, and make referrals to appropriate services.
• Advocacy and outreach: Serve as an advocate for clients in healthcare settings and participate in community outreach to identify and support clients needing services.
