JOB SUMMARY:
The Youth Wellness Hubs Ontario (YWHO)Hub Manager will lead the implementation and day-to-day operations of the Sagamok Anishnawbek Youth Wellness Hubs Ontario location. The Hub Manager will lead and supervise a multi-disciplinary team of staff who will support youth across a continuum of care.
The Hub Manager will engage and support service providers providing care, programs and services to young people through the Hub Network. This will require the Hub Manager to foster an environment of learning through data review and analysis with staff and service delivery partners with assistance from YWHO Provincial Office staff. Specific functions include, providing effective program development, monitoring and implementation of the YWHO model, handling day-to-day operational issues, leading an interdisciplinary team, preparing service reports and planning/maintaining the service program budget, working alongside community partners, youth and family members to support integrated governance of the Hub and evaluation.
The Hub Manager will support system level change that seeks to bring a continuum of accessible and integrated care for youth. The Hub Manager will engage with the provincial YWHO Network and YWHO Provincial Office and will represent the Hub leadership at the Provincial level.
The Hub Manager will facilitate and support youth-led communications and marketing activities, represent and promote the Hub Network organization in the broader community, and be responsible for ongoing and specific projects as assigned.
POSITION DUTIES:
- Plan, develop, and implement the YWHO model, continuum of care and Core Components in a way that promotes barrier-free access and is engaging for diverse youth.
- Manage the daily operations, budget, staff and community relations, administration of contracts/agreements, liaise and partner with agency administration, office and IT department as needed.
- Ensure safe operations in a busy drop-in environment and promote best practices in mental health and substance use care, social and community services and skills and wellbeing activities for young people.
- Recruit, lead, supervise and performance manage a multi-disciplinary team.
- Ensure that new staff members and partners receive orientation to the YWHO model, its operations policies and protocols.
- Manage partnership development; ensuring coordinated and collaborative system of care for youth through service implementation.
- Support service integration to ensure the continuum of care and care pathways are functioning seamlessly for youth and Hub partners by engaging young people in the development of service pathways and programs.
- Engage with service delivery partners in developing and maintaining a schedule for the Hub that offers a diverse range of supports promoting the overall wellness of youth.
- Support the collaborative development of the YWHOs strategic goals and ensure the implementation of annual business plans and special projects.
- Oversee the adoption of the YWHO data collection platform, My Wellness Passport, the use of Measurement Based Care by employees and partner agency staff.
- Oversee the adoption of the YWHO Virtual Platform as the virtual services option within the Sagamok Anishnawbek Hub, work with YWHO Provincial Office to train staff on use of the platform, and support for implementation of the platform with youth outreach and service pathways.
- Support the Sagamok Anishnawbek Hub,Youth and Family Advisory Committees, ensuring the voice, ideas, and preferences of youth, family and community members inform and direct the development and governance of the hub.
- Represent Sagamok Anishnawbek as part of the provincial YWHO Network, including attending YWHO network operational meetings, working collaboratively with the YWHO Provincial Office and the provincial Implementation and Evaluation team.
- Support and enable the access of staff and service delivery partners to YWHO Provincial Office training and capacity building activities.
- Ensure consistency among staff and service delivery partners in assigning client identifiers and overseeing data collection/data input processes, ensuring compliance and privacy within a PHIPA framework.
- Work with the YWHO Provincial Office to facilitate local evaluation data collection and selection of measures/indicators, as required.
- Contribute to the evaluation of service with respect to its effectiveness in meeting the community needs and achieving the YWHO mandate. This will include the application and use of data to inform service delivery specifically as it relates to equity and access. Submit monitoring data and reporting to the YWHO Provincial Office, as required.
TERMS AND CONDITIONS OF EMPLOYMENT:
- Respect for, sensitivity towards as well as knowledge and understanding of Anishnawbek culture, traditions, and the Seven Grandfather Teachings.
- Primarily involves evening and weekend work to effectively engage with youth outside of school hours. Flexibility is required, as work hours may vary to accommodate program needs during holidays, summer months and significant community events that occur outside of the standard schedule.
- Is subject to six months probation.
- Clear Criminal Records Check (CPIC) that is valid within 6 months.
- Valid drivers license and vehicle for on-the-job use.
QUALIFICATIONS:
- Post Secondary Degree or Diploma in Social Work, Child/Youth related or other health related field.
- Previous experience in mental health, substance use or clinical management role, with a preference for youth-focused non-profit community agencies.
- Demonstrated understanding of local community issues impacting youth, and the needs and challenges facing diverse youth and equity-deserving youth.
- Knowledge of community resources available to support youth and families across a continuum of care and holistic understanding of health and mental health.
- Experience developing and maintaining partnerships with diverse community agencies.
- Demonstrated ability to managing staff in a client-focused environment providing mentorship, and leadership and ensuring accountability.
- Extensive knowledge of approaches that promote health equity, including anti-oppressive and anti-racist practice, 2SLGBTQ+ inclusion, community engagement, cultural safety, trauma-informed care, harm reduction, and human rights and accessibility frameworks.
- Experience leading or supporting the engagement of young people and family members with lived experience.
- Familiarity with developing and implementing project-level plans and processes, as well as higher-level strategies and frameworks.
- Ability to maintain a high level of confidentiality and professionalism; familiarity with privacy legislation is an asset.
- Experience with budgeting, reporting and proposal writing is an asset.
- Excellent organizational, interpersonal and written communication skills and the ability to work effectively in a fast-paced environment are essential.
