Position Summary and Scope of Service

Reporting to the Senior Manager, Community Services and consistent with the Mission, Values and policies of St.Amant, the Coordinator, Community Services is a critical role in ensuring the success of St.Amant business and operations.
The Coordinator is responsible for the care and service provided to children and adults within the service areas and sites assigned to the position. The Coordinator has responsibilities including the provision of direct and indirect services to individuals and families. This includes ensuring that the necessary structures and processes exist to facilitate a collaborative and integrated interdisciplinary approach to the planning, delivery, evaluation and continuous improvement of services. The Coordinator supervises and coordinates services in accordance with the Mission and Values of the organization and provincial legislation and regulations.
Qualifications
Educational:
- Must possess post-secondary education in a relevant social science, rehabilitation, or health care administration
- Previous training in Project Management is desired
- Certification in Non Violent Crisis Intervention is an asset
- Previous training or exposure to Personal Outcome Measures is an asset
Professional:
- Minimum of five years' experience coordinating services to children and/or adults with disabilities preferably in a community or foster setting
- Previous experience as a manager in an interdisciplinary environment, project management and balancing multiple responsibilities and crisis resolution/counseling is an asset
- Previous experience managing budgets within a non-profit environment an asset
- Familiarity with relevant legislation (Child & Family Services Act, Vulnerable Persons Act), regulations (Foster & Approved Home licensing regulations) and standards (Foster Family Manual)
- Knowledge of the structure, regulations, and legislation of Family Services, Child and Family Services considered an asset
- Experience conducting in-depth interviews and preparing detailed reports, and previous experience conducting home studies and participating in the licensing process would be an asset
- Knowledge of Behavior Management principles and experience working with individuals that display challenging behaviors
- Must possess knowledge of theory and practice relevant to the care, education and training of people with a developmental disability or autism
- Must understand and support the concept of interdisciplinary teamwork
- Familiar and proficient in a computerized work environment (Word, Excel, Outlook, etc.)

- Able to participate in a rotational On-Call duty schedule
- Valid Manitoba Driver's License in good standing, and access to a safe, reliable vehicle
Program/Service Support
Consistent with the Mission, Philosophy and Policies of St.Amant, the Case Supervisor is responsible for managing the provision of foster care services and home share for all people within his/her caseload. Quality services are evident when professional communication, timeliness, accuracy, ethical awareness / sensitivity, confidentiality are consistently demonstrated as required by legislation and program/service standards.
Duties include:
- Reviews Meaningful Connections Logs, and conduct analysis to determine whether sufficient support, in terms of Meaningful Connection, is in place for persons supported
- Directs, trains, and supports Team Leaders/Supervisors in their efforts to support the Meaningful Connection initiative
- Provides direction and supervision to the Team Leaders/Supervisors within assigned areas to ensure services provided are of the highest quality and are directly related to the individual's assessment of their desired personal outcomes
- Leads the individual planning process for each person within caseload using a personcentred, goal-oriented approach to ensure that the overall needs and preferences of each person is addressed by an interdisciplinary team and the process and outcomes are documented
- Promotes awareness of individual rights and responsibilities within a least restrictive environment
- Provides direction and leadership to Team Leaders/Supervisors and direct care staff in crisis situations
- Collaborates with Family Services to ensure needs of people served are identified and addressed
- Participates in the development and revision of Service Contracts by assessing / identifying and articulating needs and strengths of person supported and foster providers needs
- Collaborates with Family Services / Child and Family Services to ensure individual needs are being met in a timely and efficient manner
- Identifies and assesses high-risk situations, intervenes, and documents
- Advocates for people served and their families to ensure individual or group interests are represented and/or to ensure accessibility to resources and services
- Maintains organized and up-to-date files and records
- Develops curricula and implements skill development sessions for people receiving services as needed
- Coordinates and monitors services including comprehensive tracking of activities in relation to support plan
- Assists people to develop goals and areas of need and coordinates the development of service plans which are assessed regularly
