We are seeking a dynamic, highly motivated Intensive Case Manager who demonstrates our values of compassion, accountability, responsiveness, and empowerment; to our clients, families, and each other.

The Intensive Case Manager works out of the Wesley Youth Housing program, which provides service to up to 19 youths who are homeless or at imminent risk of homelessness. They provide case management services, assessments, and light-touch counseling.

This position is a unionized full-time temporary position. The contract is until December 31, 2025, with the possibility of extension. The rate of pay is $21.62.

Key Responsibilities:

  1. Ability to establish a strong positive rapport with a diverse group of youth by being friendly, non-judgmental, non-intrusive, and approachable.
  2. Oversee the 19-unit program and residents, handling issues that may arise within a single-staffed team.
  3. Engage youth in a meaningful way by providing counseling, case management, information, support, and advocacy.
  4. Complete statistics, client files, daily logs, incident reports, Serious Occurrence Reports, and other administrative documentation, as required.
  5. Complete case manager duties such as intakes, assessments, and discharges, complete OW applications, ODSP applications, and housing applications, and submit landlord letters, and direct deposit forms with youth.
  6. Work collaboratively with community partners and ensure proper communication and sharing of information to better support our clients
  7. Manage crises as they may arise
  8. Complete household cleaning responsibilities as required.
  9. Ensure accurate and appropriate case coordination. Keep accurate/updated client files ensuring they are maintained and that challenging cases/concerns are raised at unit meetings with the manager and/or with case conferencing.
  10. Adhere to appropriate disclosures regarding confidentiality and relevant legislation.

What you will bring:

  1. Minimum of S.S.W. diploma, BSW, or post-secondary education in a related field.
  2. Member, in good standing, of the Ontario College of Social Workers and Social Service Workers, or a willingness to obtain in the first 6 months.
  3. Minimum 1 year providing direct case management, as a prime worker.
  4. Strong communication skills both verbally and in writing. French speaking would be an asset. Must be proficient with Microsoft Outlook, Word, and Excel with troubleshooting ability.
  5. Non-violent crisis intervention, positive space, ASSIST, and 1st aid/CPR training is required.
  6. Ability to work flexible hours. Working on a rotational schedule of 4 weeks of day shifts Monday to Friday 7:00am- 3:00pm and 4 weeks of afternoon shifts Monday to Friday 3:00pm to 11:00pm.
