False Creek Healthcare Centre, a division of Clearpoint Health Network, is now hiring a Patient Care Coordinator.

The Centre, located in downtown Vancouver, has three operating rooms and specializes in ENT, plastic surgery, oculoplasty, spine, gynecological procedures, and hernia surgery.

Position Summary:

We are seeking a highly organized and detail-oriented Patient Care Coordinator to support our surgical scheduling and chart preparation processes. This role is central to ensuring patients receive safe, well-coordinated care by managing medical documentation, communicating with physician offices, and supporting both clinical and administrative teams. The ideal candidate is proactive, adaptable, and thrives in a collaborative healthcare environment.

Key Responsibilities:

Medical Office Onboarding & Training

  • Onboard new medical offices, ensuring understanding of charting standards and EMR requirements
  • Provide ongoing training and support as needed

Chart Management & Documentation

  • Receive and review patient charts from surgeons' offices to ensure completeness
  • Verify inclusion of supporting medical records, test results, and labs
  • Enter charts and surgical slates into the EMR system
  • Organize and upload all relevant documents accurately
  • Maintain comprehensive and up-to-date electronic medical records

Surgical Coordination & Communication

  • Work with nursing and anesthesia teams to confirm charts are complete after surgery
  • Prepare charts for surgical day, including generating patient labels and necessary forms
  • Maintain an updated list of upcoming surgeries and follow up on outstanding documentation
  • Communicate slate changes or updates to surgeons' offices and internal teams
  • Coordinate with MDR, nursing, and admin teams to ensure all surgical slate details are accurately captured (e.g., case order, equipment needs, special notes)

Administrative Support

  • Assist reception with patient check-ins as needed
  • Help answer phones and direct calls professionally
  • Support courier coordination (incoming and outgoing)
  • Provide backup support for other admin team members during absences
  • Collaborate with leadership to improve workflows and implement process changes

Qualifications:

  • 2–5 years of experience in a healthcare administrative role, ideally in a surgical or clinical setting
  • Strong understanding of medical documentation, EMR systems, and clinical workflows
  • Exceptional communication skills, both verbal and written
  • High attention to detail and ability to manage multiple priorities
  • Collaborative team player who can also work independently
  • Proficiency with Microsoft Office and healthcare EMR platforms
  • Familiarity with medical terminology and surgical scheduling practices is a strong asset
