Job Description

CCHOHT Overview:

The Connected Care Halton Ontario Health Team (CCHOHT) is one of the first OHT teams in the province that is working to implement a new model of organizing and delivering health care that better connects patients and providers in their communities to improve patient outcomes. The Ontario Health Team (OHT) model is part of the Ontario Ministry of Health's (MOH) health system transformation strategy. The CCHOHT is guided by a Collaborative Committee with representatives from Halton Healthcare, Halton Region, Ontario Health at Home, Physicians, PFAC and Community Organizations.

This is what you'll be doing:

Reporting to the Executive Director Connected Care Ontario Health Team, the Communications and Engagement Coordinator is responsible for supporting the CCHOHT and the emerging Primary Care Network (PCN) by guiding communication and engagement activities that improve awareness of the CCHOHT and our focus on increasing attachment and access to primary care teams for 100% of all residents by 2029. Together with our partners, the CCHOHT is accountable within Ontario Health's Priorities for all OHT's to improve population-health for its attributed patient population residing in Oakville, Milton and Halton Hills.

The incumbent will work as a member of the CCHOHT to provide the following:

  • Develops internal and external communications through established & new communication channels inclusive of digital, print and in person modes.
  • Informed by Patient Family and Caregivers and OHT partners, organizing in person events within OHT partner locations focused on working with the identified needs of diverse communities to ensure health equity throughout planning and implementation of initiatives.
  • Develops regular cascades of communication through updating the CCHOHT website, social media channels and print materials inclusive of newsletters and briefing notes.
  • Liaises with all CCHOHT partners for ensuring key communications are shared and coordinated across platforms as appropriate.
  • Responsible as the first point of contact to respond to inquiries that come forward through the CCHOHT general email inbox/voicemail. Forwards media requests/escalates to the Executive Director as required.
  • Supports the development of communication materials for the CCHOHT.
  • Supports the Collaborative Committee with meeting materials, guest speakers/presentations and action notes.
  • Stays up to date on industry trends and makes recommendations for adjustments to communications strategies aligned with guidance from Ontario Health and Comparator OHT's.
  • Supports implementation of AODA standards across all CCHOHT communications.
Qualifications
  • Post-secondary degree in Communications/Marketing/Journalism/Public Relations preferred
  • Experience working within complex healthcare environments
  • 2+ years of professional experience (or equivalent) preferred
  • Experience with website design and running corporate social media accounts an asset
  • Strong writing skills
  • High level of creativity
  • Ability to think analytically
  • Detail oriented, accurate and organized
  • Proven ability to multi-task and meet deadlines
  • Excellent judgement related to communications and meaningful engagement across multiple groups
  • Demonstrated ability to work independently and collaboratively as a team member
  • Excellent communication and interpersonal skills
  • Understands the healthcare environment and provincial direction for OHT's
  • Ability to work as a team in a professional and respectful manner that embraces diversity of thought.

Additional Information

Status: Permanent Full-time

Hours: Monday to Friday - 7.5 hour day shifts (Hours are subject to change based on operational needs).

Hourly Salary: $32.700 - $40.920
