Established in 1978, SWCHC provides a wide range of health care services such as primary health care, health promotion, community development, mental health, and substance use treatments.

Description
The multicultural health navigator assists newcomer, immigrant, and refugee clients to navigate the healthcare system. The incumbent works closely with other members of the inter-professional team to provide health education, advocacy, and practical assistance while playing an intermediary role between clients and service providers within the community. The ideal candidate will understand and support Somerset West Community Health Centres mission, possess excellent interpersonal skills, community liaison ties, organizational capabilities, and reporting abilities. The individual is motivated and passionate about working with newcomers providing case management support and enjoys working in a fast-paced environment.
PositionRequirementsEducation:
Degree or diploma in social sciences or equivalent combination of training and experience
Degree or diploma in a health discipline or equivalent, an asset
Professional Experience:
Minimum of 2 years experience in a community setting with emphasis on case management, community engagement, practical assistance, and case coordination support for clients
Minimum 2 to 3 years experience working with diverse client populations, specifically with newcomers, refugees, and immigrants in Arabic and French communities.
KeyCompetencies:
Minimum two years experience in a community-based setting
Minimum three years experience working with multicultural population.
Knowledge of the Canadian heath care system & social determinants of health
Basic health assessment and interventions, as appropriate
Sensitivity to and passionate about multicultural issues
Awareness related to cultural competence.
Knowledge of related community resources
Ability to liaise with external parties and to advocate for clients and provide home visits as necessary.
Commitment to strength-based philosophy and practice
Assessment and intervention skills; sound judgment
Ability to provide basic education to individuals and families.
Excellent communication skills, able to bridge gap between clients and service providers/service agencies and advocate for clients.
Strong interpersonal and organizational skills
Ability to work independently and liaise with internal and external partners.
Must be able to work on multiple tasks simultaneously to meet deadlines.
Respect the privacy of all clients and adhere to strict confidentiality protocols to protect client personal and sensitive health and social information.
Ability to work evenings and weekends, as required.
Proficient use of computers and various software applications, context and understanding of Electronic Medical Record software
