SERVICE COORDINATOR

Full Time Contract (11 month Maternity Leave)

South-East Ottawa Community Health Centre is a community health centre, offering a variety of health and social services to residents living in the South-East Ottawa Community Health Centre catchment area. We require a bilingual (French/English) Service Coordinator to work with our Social Services team.

JOB SUMMARY

The Service Coordinator is a member of the Social Services team and is responsible for the implementation of Community and Social Services (CSS) programs including, but not limited to, Home Help Directory, Caregiver Supports, Handy Helpers, Friendly Visiting, Grocery Supports, Grocery Bus, LunchAbility, Telephone Assurance, Transportation to Medical Appointments, and SnowGo Assist .

POSITION REQUIREMENT

  • Education: Post-secondary education in Social Sciences or an equivalent combination of education and related experience.
  • Professional Experience: Background in gerontology and experience working with adults with disabilities, with strong skills in assessing physical, psychological, social, recreational, and spiritual needs.
  • Key Competencies:
    • Knowledge of volunteer community organization operations.
    • Strong outreach, program development, and group facilitation skills.
    • Excellent written and verbal communication abilities.
    • Solid understanding of community resources.
    • Ability to conduct meetings, direct home support workers and volunteers, and work independently or as part of a team.
    • Strong time management and computer skills.
  • Language Skills: Proficient in English and French (oral expression, listening, reading, and writing at an "A" level).

RESPONSIBILITIES

  • Assess and Support Clients: Conduct client assessments (seniors, adults with disabilities, caregivers), provide information, make referrals, and offer individual support as needed.
  • Coordinate Home and Volunteer Services: Recruit, screen, orient, and supervise home support workers and volunteers; maintain ongoing contact to ensure effective service delivery.
  • Develop and Deliver Programs: Plan and implement training sessions, peer support groups, outreach activities, and community events based on community needs.
  • Information, Referral, and Advocacy: Provide clients with information, advocacy services, and connection to appropriate community resources.
  • Collaborate with Staff and Partners: Work closely with other service coordinators, staff, and community organizations to deliver joint programs and ensure service continuity.
  • Administrative Duties: Maintain accurate records, generate service reports, develop program procedures, and contribute to data collection and reporting activities.
  • Promote Programs and Community Engagement: Represent and promote the Centres services through public events, advertising, and partnerships with other agencies.
  • Ensure Health, Safety, and Professional Standards: Comply with occupational health and safety requirements, uphold confidentiality, participate in training, and engage in ongoing professional development.

SALARY RANGE: $47,902.40 to $56,347.20 per year depending on experience
