Job Description

As part of the Patient Flow Team, will implement system wide initiatives to facilitate the ease of patient movement through the hospital system. Working collaboratively with all teams, you will identify and resolve barriers to ensure effective flow of patients. This is a non unit producing position with administrative bed allocation responsibilities and authority to determine all bed assignments.

  • Provides direction and coordination of admissions, discharges and transfers and has knowledge of current bed availability across the entire hospital
  • Chair daily bed meetings with the CRNs, Patient Care Managers, Staffing Resource Team and additional relevant stakeholders
  • Act as a liaison between units to ensure smooth, timely and appropriate placement of admitted patients
  • Ability to assess risk and support patient acuity, while considering the global needs of all patients and hospital programs
  • Regularly reviews preferred accommodation bed requests and follows up with the units to ensure accommodations are achieved whenever possible
  • Understand demand and capacity principles and processes and works in partnership with unit staff to identify/remove barriers to help resolve challenging situations
  • Develop and maintain strong, effective partnerships with all members of the health care team
  • Identify and track various Patient Flow performance metrics
  • Responsibility for tracking use of over census beds using developed tracking tool
  • Liaises with ICU, Paediatrics, Maternal Child, Mental Health and Addictions and other specialty areas that arrange their own repatriations and utilizes this information to make appropriate placement decisions
  • Conducts regular rounds in the Emergency Department and In-Patient Units to facilitate the smooth transition of admissions, maintain visibility, and determines alternate options for bed placements to mitigate congestion in ED and other admitting sources
  • Work in conjunction with Decision Support, Manager of Patient Flow and Director of Patient Flow to track identified metrics
  • Contribute to the overall goal of timely patient transfer requests include life or limb Criticall, repatriations, inter-facility transfers and compassionate requests. Responsibilities include contacting external facilities to follow up on existing incoming/outgoing repatriations/bed offers, and to initiate/coordinate new repatriation requests, particularly on weekends.
  • Assess and make recommendations for changes in current practice as per Halton Healthcare focus on continuous quality improvement
Qualifications

  • BScN degree, Masters preferred
  • Current member in good standing with the Ontario College of Nurses
  • A minimum of 3-5 years progressive work experience in a patient flow/leadership role in an acute care health care setting
  • Proficiency with Meditech Electronic Documentation System and Visibility required
  • In-depth knowledge of individual sites and multi-site organizational processes required
  • Demonstrated experience in the development and implementation of best practice initiatives
  • Proven ability to develop and maintain effective working relationships with multiple partners
  • Proven ability to effectively influence change in a complex multi-site healthcare environment
  • Ability to listen, engage, collaborate, negotiate, facilitate and entertain creative solutions in order to enhance patient flow organizationally
  • Utilizes the Surge Policy to support the identification of an appropriate strategy and plan of action to prevent or deal with gridlock
  • Excellent organizational, interpersonal and problem-solving skills
  • Ability to cultivate positive working relationships with internal and external stakeholders
  • Ability to manage high job demands in a fast-paced environment and be self-directed
  • Ability to think critically and respond to a rapidly-changing environment with competing demands. Maintains a level of fluidity or flexibility to be able to deal with new information and/or unplanned situations
  • Must be physically able to round on units on a regular basis to assist in removing barriers to timely patient transfers. This may include engaging with appropriate physicians and health team members to help alleviate patient flow barriers/pressures
  • Aid in optimize resource utilization (including staffing resources) within the organization, re-organizing staff where appropriate to ensure safe and efficient workflow
  • Satisfactory employment and attendance record

Additional Information

Hours: Monday to Sunday, various rotating 12 hour shifts, 0700-1900 (Hours may be subject to change based on operational requirements)

Salary: $50.00 to $61.93 per hour
