Job SummaryAs a valued RPN member, you will be a part of an amazing team, performing a variety of tasks tailored to the individual needs of our clients. Your primary responsibilities will include conducting assessments within the community and collaborating with the interdisciplinary team to develop comprehensive care plans. We are looking for a casual part-time RPN, in Toronto and Etobicoke someone who has a minimum of 5-10 hours a week throughout the week and/or weekend.
Responsibilities:
  • Established comprehensive client assessments, develops care plans, implements and evaluates care based on client's needs/preferences
  • Completes and maintains related records and documentation
  • Develops standards that ensure safe and effective services to clients and families;
  • Ensures that the level of care established for clients is safe and appropriate based on client and home assessment
  • Directs the implementation of service goals and objectives
  • Ensures the evaluation of client care services
  • Ensures standards of care, compliance with College of Nurses standards, state/provincial regulations and standards of care
  • Ensures a high standard of customer service is delivered to clients from intake to client conversion, case setup- up and delivery of services
  • Collaborating with the families, healthcare professionals, and the team
Qualifications
  • Registered with the College of Nurses of Ontario (CNO) in good standing
  • Experience in doing assessments in Home and Community Care ( 1-2) years
  • Current BCLS certification (Level C)
  • Demonstrated clinical assessment and patient/family teaching skills
  • Experience with seniors with cognitive impairment
  • Proven excellent interpersonal skills and effective oral/written communication skills
  • Demonstrated excellent organizational, time management, prioritization problem-solving, and conflict-resolution skills
  • Demonstrated planning and coordination abilities
  • Proven ability to work effectively as part of a team
  • Demonstrated ability to work with minimal supervision
Requirements:
  • Clear & updated Vulnerable Sector Check (within the last six months)
  • Valid Drivers License & reliable car
  • Up-to-date TB test within the last four years
  • High level of customer service experience
  • Community Knowledge/Experience
  • Clinical expertise ( creating care plans, assessments, etc.)
  • Ability to integrate and apply critical thinking skills to client care
  • Proven interpersonal, organizational, written, and verbal communication skills.
  • Strong knowledge of best practices for dementia, delirium, depression ( preferred)
  • Ability to work independently in a fast-paced, diverse, and multicultural environment.
  • Demonstrated commitment to providing the highest level of satisfaction in client and staff service care
