POSITION SUMMARY

Reporting to Program Manager, The Tower Case Manager is responsible fora caseload of tower residents, including goal planning, tracking and reporting monthly statistics, client assessments, and collecting rental arrears. The individual in this role will develop a detailed understanding of the Tower Housing programs' operations and will assist the team with effective client care.

The role of the Tower Case Manager is to take charge of their caseload and also support in other areas of programming to drive efficiency and effectiveness. This position fills a case management role in the Tower Department and is actively involved with the Calgary Dream Centre's program projects.

RESPONSIBILITES

  • Carry and manage a case load of up to thirty resident clients
  • Oversee the Case Management team providing supervision, training, and guidance
  • Train and support new staff when required
  • Act as the point person for Tower related issues
  • Assist clients with goal planning, providing support and referrals according to their needs
  • Mentor client's growth and oversee their progress and interventions while providing advice, support, and direction
  • Involve the Program Manager in all client meetings that may lead to eviction
  • Uphold program delivery standards with Program Manager to ensure best practices are followed
  • Responsible for ensuring client maintains a healthy and safe transition throughout the programming
  • Responsible for attending weekly meetings to report client's rental status to Property Management Team
  • Follow up with clients regarding unhealthy room states and/or non-compliance in program duties
  • Track and report client progress to analyze, assess and review objectives
  • Incorporate regular written case notes for clients, as well as create relapse prevention plans relevant to all client cases
  • Facilitate meetings, groups, and classes as needed
  • Submit and forward Alberta Works applications on behalf of clients for income support
  • Report to Program Manager about the clients' successes and challenges daily in writing
  • Attend resident graduations on a weekly basis
  • Other duties as assigned

EXPERINECE AND EDUCATION REQUIRED:

  • Education in social work, Psychology, or related social science is an asset
  • 1-3 year's experience working in the field of addiction and with marginalized populations To transport clients, a valid Driver's License and 2 million liability insurance is required

DESIRED SKILLS AND ABILITIES:

  • The ideal Tower Case Manager demonstrates and excels in leadership and effective communication skills. Additional skills required in this role include:
  • Comprehensive understanding of the challenges from the experiential perspective of men relating to poverty, addictions, violence, and mental health
  • General knowledge of 12 Step and other recovery-based model
  • Ability to effectively manage confrontations and conflict
  • Self-starter with excellent organizational skills, including strong problem-solving, and analytical skills; able to manage priorities and workflow, and possess excellent time management skills
  • Knowledge of local social service agencies and referrals
  • Able to prepare comprehensive reports, and maintain client case files
  • Ability to work with minimal supervision, take initiative and be self-motivated
  • Acute attention to detail and a commitment to excellence and high standards
  • Experience using HMIS is an asset

As a condition of employment, all staff are required to produce a clear Criminal Record Background check with a Vulnerable Sector search upon hire and every three years thereafter.
