Position Overview

This is a Hybrid role, reporting to the Area Director of Operations, the successful candidate will provide clinical oversight to a Home Care direct service team across Ontario. The Clinical Practice Lead will collaborate with the concierge team and other key stakeholders to ensure that members receiving service are provided with the highest quality consistent support throughout their care journey with Home Concierge. The Clinical Practice Lead will also help to drive expansion opportunities including some business development and community outreach for the nursing service. This role is eligible for a performance incentive bonus. Clinical outcomes along with targeted expansion activities for the nursing service will form part of the metrics against bonus eligibility. Travel across the GTHA may be required to meet the needs of the members receiving care.

Responsibilities:

  • Complete Initial assessments for members new to service.
  • Complete reassessment of members with changing medical status.
  • Lead the management of PPE distribution, infection control, policy and practice.
  • Ensure Mask fit testing is completed for direct team members.
  • Complete nursing visits to ensure success and satisfaction due to staffing gaps, vacation or absence.
  • Drive the nursing business line against company metrics.
  • Promote the nursing service at community events.
  • Collaborate with the Manager of Business Development with respect to strategic partnerships.
  • Management of incident reporting system, investigations, and outcomes.
  • Provide Case Management to members receiving service.
  • Review, implement and manage the compliance of policies relating to clinical practice and outcomes.
  • Completion of monthly chart audits.
  • Provide clinical support with RFP, or new business activities.
  • Lead the creation of clinical education and standardize practice of delegation training to front line team members.
  • Create and maintain repository of standard operating procedures as it pertains to clinical practice.
  • Other duties as assigned.

Requirements:

  • Current Certificate of Competence with the College of Nurses of Ontario.
  • Bachelor of Science in Nursing required.
  • A minimum of 2 years of recent community nursing experience is essential.
  • Certification or equivalent experience of working with older adults.
  • Up to date with clinical practice including infusion therapy.
  • Ability to adapt to new technologies, including telephone systems, slack, teams, and other computer apps.
  • Excellent communication skills, both written and verbal.
  • Demonstrate ability to use critical thinking skills and adapt to a changing environment.
  • Demonstrated ability to work independently and to act as a collaborative team member.
  • Experience in leading quality improvement processes.
  • Flexibility with respect to scheduling and an ability to participate in an on-call rotation.
  • Vehicle and valid driver's license, with the ability to travel and attend in-person, when required.
  • Updated immunization records including Covid 19, clear criminal background check, and Vulnerable sector check are required upon hire.
