Woolwich Community Health Centre (WCHC) is hiring a Nurse Practitioner at a permanent part-time level (1.0 FTE, 37.5 hours/week), with the option of going up to full-time on an as needed-basis. The NP will provide day and evening coverage for our Wellesley and Linwood sites. All WCHC Nurse Practitioners work to their full scope of practice within an inter-disciplinary team to provide comprehensive primary health care to a registered client caseload. In addition to providing comprehensive primary care, within this unique role, the Nurse Practitioner will also engage in community-facing programing aimed at reducing primary care unattachment rates across rural Waterloo Region and surrounding areas. This role is most suited to candidates who excel within full-scope practice, and are motivated to nurture skills of interdisciplinary leadership, creative healthcare solutions, and outcomes-based primary care program development.

This role is most suited to candidates who are comfortable with the full scope of the RNEC role, confident with their own skill set, self-motivated, and able to practice independently, as there may be occasions when a physician is not on-site.

Requirements:

  1. BScN with Primary Health Care Nurse Practitioner certificate and registered with The College of Nurses of Ontario in Extended Class. Registered Nurses with significant, relevant previous work experience, who are planning to write their RNEC exam within the next 6 months, will be considered.
  2. Broad-based nursing experience with 2-5 years in primary health care setting.
  3. Commitment to working as part of an inter-disciplinary team that includes family physicians, nurse practitioners, clinical nurses, chiropodists, registered dietitians, counsellors, physiotherapy and community health team staff
  4. Skills/experience to fulfil major functions of role: wellness care; assessment, diagnosis and management of episodic illness; managing chronic disease; and group work.
  5. Strong communication, organizational, and issue management skills.
  6. Excellent interpersonal and problem-solving skills, within the inter-disciplinary team setting and with clients and community members.
  7. Flexibility to work one evening per week as well as provide after hours on-call on shared schedule with 15 other health centre physicians and nurse practitioners.
  8. Knowledge of/experience with health issues across the lifecycle. Specific expertise in the following areas would be considered an asset: geriatrics, palliative care, mental health, chronic disease management, maternal and infant/child health as well as familiarity with working in a rural setting.
  9. Strong facility with computer; previous experience with electronic medical records would be an asset.
  10. Car required for home visits.

Compensation & Benefits:

  • Salary of $101,905 to $122,178 per year (pro-rated to working status) ($52 to $63 per hour)
