Position summary

Under the supervision of the Recovery Home Supervisor, the Case Manager will operate efficiently in accordance with the philosophies, policies, and objectives established by Seabird Island Band Management and within the specifications and regulations of the Community Care Facility Act.

The primary goal of the Case Manager is to support residents with a caring and supported environment in which to address addictions and substance use. Through a variety of programs, the Case Manager will ensure that residents can access health, healing and other services and programs, through programming in the home and through broader community services, to address the underlying causes of addiction and substance use, and to make choices that are life sustaining and positive.

What you'll do

Resident support

  • Overall leadership, including individualized wellness plans,
  • One-to-one case work with residents, including referrals to counsellors, employment, and education.
  • Coordinate services for residents who need permanent housing, health care, financial help or job training.
  • Assess struggles with domestic violence, physical or emotional illness, alcoholism, drug abuse.
  • Daily group leader – addictions, trauma care, anger management, family dynamics, harm reduction, peer support etc.
  • Cultural and community connectivity.
  • Life skills / vocational planning

Administration

  • Provide monthly reports to the Health and Social Development Director, containing both narrative and financial information.
  • Manage the home's daily operating budget (groceries, clothing or other emergency needs).
  • Ensure that the home meets regulation covering health, fire, safety requirements.
  • Maintain highly confidential, professional, and accurate resident files.

Facility

  • Plan and follow up with staff to ensure a strategic cleaning routine is completed daily/weekly/monthly to ensure health and safety and licensing expectations.
  • Ensure SIB Maintenance/Public Works is aware of the specific needs, including specific cleaning and sanitation needs, routine maintenance expectations, and any maintenance/repair needs to ensure the safe operation of the facility and programs.
  • Ensure all equipment and facilities are always in safe and proper working order

Programs

  • Ensure that all programs are meeting or exceeding the expectations for resident and employee health and safety, including executing regular fire and earthquake drills.
  • Ensure that residents are able to access culturally appropriate programing and services inside and outside the home to meet their physical, social, emotional, creative, and cognitive needs.
  • Ensure that residents have individualized plans on file, and that they are always kept current.
  • Other duties or responsibilities as assigned by your Supervisor, Manager, Director, Executive Director, or Chief & Council.

What you bring

Education

  • Certificate as Addictions Support Worker, or Diploma from a recognized Social Services Support Worker educational program, with special emphasis/experience in First Nations individuals, families, groups, communities and culture.
  • Food safe Level 1 is required.
  • Valid first aid certificate is required.
  • Managing Hostile Interactions, Non-Violent Crisis Intervention certificates considered an asset.
  • Mental Health First Aid, Naloxone, Trauma Informed training is required.

Skills/Experience

  • Professionally grounded in First Nations spirituality, philosophies, ideology, knowledge, and methodologies in approach to case management and social work
  • Minimum 3 years' related case management experience in a residential or program setting
  • Ability to establish and maintain rapport with residents.
  • Ability to observe and recognize changes in residents.
  • Ability to deal effectively with conflict situations, manage aggressive behavior, and use non-violent crisis intervention and self-regulating techniques.
  • Ability to work independently and in cooperation with others.

Other

  • Proficient computer skills with the Microsoft Office 365 Suite (Outlook, Word, Excel, PowerPoint, Teams.)
  • Proficient interpersonal and communication (written and verbal) skills.
  • Ability to agree as well as adhere to Seabird Island Band Human Resources (HR), Finance, and Information Technology (IT) Policies and Procedures, etc.
  • Knowledge, respect and understanding of Stó:lō culture, traditions and language or willingness to learn.
  • Physical and emotional ability to carry out the duties of the position.
  • Experience working with First Nations community is considered an asset.
  • Valid BC Driver's License with a clean Driver's Abstract within the last 5 years.
  • Satisfactory Criminal Record Check.
