As a member of the Community Development team, the Social Prescribing Community Connector (SRxCC) is a link between Guelph CHC's clients, primary care and allied health staff and internal and external community resources. Clients receive social prescriptions (SRx) for non-medical needs and the SRxCC assists with accessing social and community programs and services.

This role provides information, practical assistance, social support, and problem solving to help clients overcome barriers associated with the social determinants of health, with a focus on reducing social isolation. The SRxCC acts as a resource to Guelph CHC staff, ensuring staff have the support required to make SRx. The SRxCC also provides support to community programs and completes health promotion administrative tasks.

PRINCIPAL JOB DUTIES

  • Providing information, assistance, support, and/or accompaniment to help clients access community resources
  • Establish and maintain knowledge of internal and external community resources, programs and services.
  • Participate in health promotion and SRx quality improvement initiatives, research, data collection and evaluation as needed.
  • Support the Health Promoter with the agency's broader SRx initiative and special projects as needed.
  • Build relationships with primary care and allied health staff, community partners and attend relevant meetings to expand and promote SRx
  • Support, facilitate and/or co-facilitate community programs.
  • Complete health promotion and SRx administrative tasks, eg. provide program updates to communications staff, maintain participant lists, program cancellations as needed.

KNOWLEDGE, SKILLS & ABILITIES

  • Three to five years' experience in a community setting in a health or social service organization.
  • Work through an anti-oppressive lens with experience working effectively with culturally, economically and socially diverse clients.
  • Strong knowledge of local community and social resources.
  • Understanding of health promotion, community development and SRx.
  • Use of an assets base lens and experience with motivational interviewing.
  • Strong attention to detail, organizational skills and ability to work independently.
  • Effective time management skills including the ability to multitask, work within set timelines and to work independently.
  • Strong problem solving and decision-making skills.
  • Proficiency in the use of technology and various software programs
  • Possession of a valid Class “G” Ontario Driver's License, access to a vehicle and willingness to travel to community organizations as needed.

Preference will be given to applicants who have lived experience using local community and social resources, and who currently or previously identify with the equity deserving group(s) that Guelph CHC serves such as those who are; homeless or at risk of homelessness, low income, newcomers to Canada with language barriers, moderate to severe mental health/addiction issues and/or disabilities, without access to primary care, Indigenous communities, 2SLGBTQAI+ populations, families experiencing violence, family conflict, social isolation and attachment struggles.
