Job Description: Reporting to a Manager in Primary Health Care, in the North Network, the Chronic Disease Management Diabetes Nurse Educator responsibilities include; coordinating education for clients living with diabetes, providing best practice in diabetes management as part of a multidisciplinary team, providing individual inpatient and outpatient education for clients, facilitating group classes in the outpatient setting, promoting self-management education and participating in departmental activities such as program development, planning, quality improvement and research.

Experience

  • 5 years recent nursing experince
  • 3 years clinical experience with patients/families with diabetes disease mgmt
  • 1 yr experience in adult education/group facilitation
  • Experience working with complex patients and families

Education

  • Bachelor of Science in Nursing (BScN)

Licenses

  • Valid Class 5 driver's license

Other Education and Training

  • Current Certification as a Diabetes Educator (CDE) is required
  • Basic Life Support Certificate (BLS)

  • Basic - Teaching skills

Knowledge and Abilities

  • Demonstrates advanced assessment and intervention skills.
  • Ability to teach staff and patients/residents/clients/families

  • Communication skills to transfer knowledge/skills

Other Information

  • Professional membership with Diabetes Canada is desired
  • Must possess a valid driver's license and have a vehicle available for work.
  • Registered/eligible with the College of Registered Nurses of Saskatchewan (CRNS)
