Job Description: The Assessor Coordinator – Degree provides full case management services under managerial direction within an interdisciplinary team. This role is patient centered, flexible, cost effective, and quality driven, focusing on advocating for clients and coordinating resources based on assessed needs. Working in Primary Health Care, the position serves as an entry point for accessing services such as community, home, long term, and acute care. Key duties include intake, engagement, care planning, monitoring, evaluation, and discharge. The role collaborates with healthcare providers, families, caregivers, and agencies to assess needs, coordinate services, and ensure timely, appropriate care delivery.
Human Resources Exemption: No Education:
- Bachelor Degree in a related Health Science or Social Science discipline

- Ability to facilitate applications/participate in integrative case management
- Demonstrates ability to manage time and competing deadlines
- Sound clinical judgement skills.
- Ability to establish and maintain interpersonal/consultative relationships
- Advocacy, negotiation, crisis management and conflict resolution
- Responsible for accurate observation/documentation of patient related info.
- Ability to develop educational resources and presentations
- Has demonstrated strong interpersonal and communication skills.
