Job Description: The Assessor Coordinator – Degree provides full case management services under managerial direction within an interdisciplinary team. This role is patient centered, flexible, cost effective, and quality driven, focusing on advocating for clients and coordinating resources based on assessed needs. Working in Primary Health Care, the position serves as an entry point for accessing services such as community, home, long term, and acute care. Key duties include intake, engagement, care planning, monitoring, evaluation, and discharge. The role collaborates with healthcare providers, families, caregivers, and agencies to assess needs, coordinate services, and ensure timely, appropriate care delivery.

Education

  • Bachelor's degree in a related Health Science or Social Science discipline

Knowledge and Abilities

  • Has demonstrated strong interpersonal and communication skills.
  • Ability to appropriately intervene/attain intervention in emergency situations
  • Ability to facilitate applications/participate in integrative case management
  • Ability to develop educational resources and presentations
  • Demonstrates ability to manage time and competing deadlines
  • Demonstrates empathy, trust and respect in the provision of care
  • Knowledge of social determinants of health affecting vulnerable populations
  • Advocacy, negotiation, crisis management and conflict resolution
