Job Description: The overall function of the assessor/coordinator (client care coordinator) position is to assess and coordinate single entry access to personal care homes and long term care. "Preference for this position will be given to HSAS members in accordance with the terms and conditions of the SAHO/HSAS Collective Agreement"

Education

• Bachelor’s degree in nursing or social work
• Must have a current practicing registration with Saskatchewan Association of Social Workers or College of Registered Nurses of Saskatchewan.

Other Education and Training

• Requires a valid driver's license.

Knowledge and Abilities

• Knowledge in the SHA, community resources and applicable programming

Other Information

• Travel required
• Registered/eligible with the Saskatchewan Association of Social Workers (SASW)
• Previous Client Assessment Experience is an Asset
