• Provide expert knowledge and professional practice leadership to develop service delivery models for seamless transitions and integrated care
• Assess and ensure operational readiness and capacity plan to support implementation and delivery
• Lead development and operationalization of processes, practices, workflows and resource plan to support new programs and opportunities without impacting existing service delivery and patient care
• Support the strategy, development, execution and evaluation of new integrated care models with our health system partners
• Contribute to the development of new models of service delivery and operational efficiencies to improve patient outcomes and experiences
• Build and maintain key relationships with internal and external partners to identify and seize opportunities to integrate bundled service models into operations
• Develop, lead, coach and mentor program teams in the service delivery planning and execution of new models of care
• Develop, monitor and assess key performance indicators for the programs and initiate quality improvement processes and changes as required

• Undergraduate degree in Nursing with 5-7 years of leadership and management experience in healthcare organizations
• Strong knowledge and experience in healthcare operations, change management and program development with multiple stakeholders in complex environments
• Registered Nurse (RN) in good standing with the College of Nurses of Ontario
• Master's degree in Nursing, Business or Health Administration preferred
• Strong knowledge of Ontario's health system design, funding models and an understanding of priority issues facing local / regional health systems

The Program Director leads the development and management of specialized care programs that bridges the collaboration between Spectrum's Integrated Care team and health system partners with our operational and service delivery teams to plan, execute and operationalize newly designed integrated care models. The Program Director is an innovative nurse leader that champions change and develops and maintains key relationships with health system partners and clinical leaders to contribute to the design of integrated care models.
