The Home Care Specialist, Hospital to Home program cultivates and manages high-performing teams comprised of Nurses, Personal Support Workers (PSWs), and Allied Health professionals – fostering a collaborative, innovative, and learning-focused environment that aligns with Spectrum's Mission, Vision, and Values.

In this role, you will ensure seamless program operations, leveraging data-driven insights to optimize service delivery, enhance quality, support team members to work to full scope of practice including indirect care coordination activities, and achieve key outcomes. You will work alongside your integrated care team to ensure the care provided remains centred on the patient, family, and care partner's goals of care.

The Home Care Specialist must be available for days Monday through Friday and available to participate in On-Call rotation after hours and weekends.

  • Lead an integrated care team, managing day-to-day coordination of services ensuring continuity of care, seamless processes and optimal patient and employee experience
  • Collaborate with your team to assess, plan, and coordinate with patients, caregivers, and other members of the care team to establish, review, and revise care plans that support patients.
  • Oversee and monitor indirect care coordination activities provided by regulated and unregulated health professionals on your care team
  • Identifies learning needs of regulated and unregulated care providers and collaborates with internal team and hospital/community partners to prepare and deliver the training content.
  • Motivates, directs and coaches regulated and unregulated care providers to ensure high quality of care consistently meets defined organizational standards.
  • Responsible for the management of regulated and unregulated care providers, conducting performance appraisals and implementing progressive disciplinary action.
  • Responds to complaints, compliments, and patient referral issues to facilitate an effective resolution.
  • Support patients and their caregivers, ensuring that their questions are answered and a seamless care approach is in place. Build a trusting relationship to understand the patient goals
  • Effectively build and manage key internal and external stakeholder relationships
  • Address quality, safety, and risk issues, developing improvement plans as needed and participate in Quality-of-care Reviews as required with partners.

Qualifications
  • Registered Nurse (RN) in good standing with the College of Nurses of Ontario.
  • Minimum of 5 years nursing experience in a community setting and 2+ years in a management role
  • Experience with program development, implementation and evaluation
  • Experience managing teams in a unionized environment an asset
  • Experience working with care coordination
  • Valid G driver's license and reliable vehicle
