- Leads the development, implementation and evaluation of QI projects to enhance home care services
- Uses data-driven approaches including PDSA cycles, Lean, Six Sigma, clinical indicators and patient experience data to identify areas for improvement and drive measurable outcomes
- Data analysis and performance monitoring including reporting on key performance indicators, patient satisfaction and safety metrics as well as monitoring compliance with accreditation and regulatory standards related to clinical quality
- Provides education, training and coaching to home care staff on quality improvement methodologies and best practices
- Develops and updates policies, protocols and guidelines in alignment with best practices in home care quality and safety
- Collaborates with key stakeholders including interdisciplinary teams, Patient and Family Advisory Committee and external quality councils
Qualifications
- Bachelor's degree in Nursing, Public Health, Health Administration, or a related clinical discipline. A Master's degree preferred.
- 3-5 years in healthcare quality improvement, with experience in home care, community health (preferred)
- Certifications: LEAN, Six-Sigma, or other QI Methodologies is an asset; Healthcare Quality or Patient Safety
- Proficiency in using QI tools and software
