Responsibilities
Provide primary health care services to patients of all ages in a CHC model of care including health assessments and diagnosis, health care management and therapeutic interventions, screening, referrals, education, follow-up as well as health promotion and the prevention of illness and injury in accordance with the Standards of Practice for Nurse Practitioners.
Collaborate with members of an interdisciplinary team to plan, implement, co-ordinate and evaluate patient care in consultation with patients and their families.
Participate in the development, delivery and evaluation of educational and health promotion programs within the community as needed.
Qualifications
Bachelor of Science in Nursing
Registered Nurse in the Extended Class (as recognized by CNO)
Masters in Nursing, preferred
Preference of experience in a community health or primary care setting
CPR and First Aid
Knowledge and experience in health promotion, education and program evaluation
Knowledge of community resources especially in health and social services
Ability to work within an interdisciplinary team
Comprehension of French and English preferred
Computer knowledge

The Tuition Support Program for Nurses (TSPN) offers tuition reimbursement to recent nursing graduates from rural and remote communities who are new College of Nurses of Ontario (CNO) registrants and who choose to do a return-to-service (ROS) in an eligible underserviced community. The program is open to nurse practitioners who apply one year of having graduated from a Canadian University or College.
